Preoperative Instrumental Activities of Daily Living Predicts Survival After Transcatheter Aortic Valve Implantation

Background:This aim of this study was to clarify prognosis after transcatheter aortic valve implantation (TAVI) in patients with aortic stenosis (AS) and to identify baseline factors associated with mortality.Methods and Results:We prospectively enrolled 257 consecutive elderly persons with AS who were referred to Keio University Hospital and who underwent assessment of cardiac, physical (walking speed), cognitive, and renal functions, nutritional status, activities of daily living (ADL), instrumental ADL (IADL) assessed with the Frenchay activities index (FAI), and comorbidities. The primary outcome was postoperative death. Differences in basic characteristics were compared between a group that survived for a median of 661 days (IQR, 0–1,289 days) after TAVI and a group that did not. Multivariate hazard ratios (HR) were calculated for independent factors selected in Cox proportional hazard models. Thirty-one individuals died during follow-up. Walking speed was significantly faster (0.87±0.25 vs. 0.70±0.24 m/s, P<0.001) and FAI was significantly higher (21.2±8.0 vs. 15.7±8.0, P=0.026) in the survival group compared with those who died. Multivariate HR for mortality according to walking speed was 0.05 (95% CI: 0.028–0.091) in model 1 and 0.04 (95% CI: 0.020–0.081) in model 2, and those for FAI were 0.94 (95% CI: 0.92–0.95) and 0.92 (95% CI: 0.90–0.92), respectively.Conclusions:Preoperative walking speed and IADL are crucial factors associated with prognosis after TAVI even after adjustment.

The Smart Life Stay (SLS) program: effects of a lifestyle intervention program in combination with health tourism and health guidance for type 2 diabetes

Background: The aim of this study was to determine the effectiveness of the Smart Life Stay (SLS) program, which is an experience-oriented stayover program, in combination with health tourism and mandatory health guidance on glucose metabolism after 2 years. Methods: The participants of the SLS program (n = 792) were recruited from a database of 23 medical insurers. They underwent a mandatory health examination termed Specific Health Checkups in 2014. The participants were included if they had diabetes or were at a high risk of diabetes and if they satisfied the following inclusion criteria: (1) body mass index (BMI; kg/m2) > 25, or (2) waist circumference (WC; cm) > 85 for men and > 90 for women, or (3) hemoglobin A1c (HbA1c; %) > 5.6, or (4) fasting plasma glucose (FPG; mg/dl) > 100. Individuals who corresponded to one or more items were included as study participants. The control subjects (n = 3645) were nonparticipants of the program who were selected from the database and met the inclusion criteria. The lifestyle changes and changes in mean BMI, WC, FPG, and HbA1c in both groups from baseline to 2-year follow-up were compared by inverse probability weighting of a propensity score. Results: The percentage of people who exercised regularly increased significantly in the SLS group compared with the control group. In the SLS group, BW, BMI, and WC significantly decreased by 1.75 kg, 0.60 kg/m2, and 1.45 cm, respectively, whereas in the control group, WC, FPG, and HbA1c increased significantly by 0.38 cm, 3.37 mg/dl, and 0.12%, respectively. The comparison between groups revealed that the BW, BMI, WC, FPG, and HbA1c improved significantly in the SLS group. Conclusions: The SLS program is suggested to help improve glucose metabolism. This program could be a feasible option as a lifestyle intervention program for diabetes.

The role and attitude of senior leaders in promoting group-based community physical activity: a qualitative study

© 2020 The Author(s). Background: In the context of worldwide public health, it is very important to promote physical activity among the older people. This study explored the roles and attitudes of senior leaders in promoting group-based exercise in their local communities, specifically to determine the level and extent to which to elderly participation was encouraged. Methods: This study conducted semi-structured face-to-face in-depth interviews and employed a subsequent thematic analysis. Participants included 10 club leaders and five sub-leaders who were working at senior clubs in Fujisawa-city, Kanagawa, Japan, from July to September 2018. Results: Four themes emerged from the interview responses, including "unwavering attitude/conviction in relation to the vision,""leaders must set an example,""a search for balance in delegating responsibilities to members,"and "creating and fostering culture and environment of mutual help."Further, each participant outlined several aims, including "achieving healthy longevity for the entire local community,""having older people promote healthy activities among the older people,"and "creating a pro-health town."Conclusions: Findings indicate that policymakers, public health workers, and healthcare providers should recognize the pivotal roles that senior group leaders play in promoting healthy activities for the older people. These efforts should be strongly considered when developing policies and strategies designed to promote overall healthy longevity from a general community perspective.

近年、小学校の温熱環境が児童の学習効率や健康状態に影響を及ぼすことが報告されている。また、温冷感には代謝量が影響しており、性別、年齢、活発さによる個人差が生じる可能性がある。以上より、温熱環境が児童の健康を維持するために重要な要素であり、その評価には、基礎代謝量や身体活動習慣の個人差を考慮することが必要であると考えられる。本研究では、小学校における温熱環境と温冷感・諸症状の関連について、身体活動を考慮した検討を行った。

Global Action Plan on Physical Activity 2018–2030: considering Sustainable Development Goals and Olympic legacy

Although physical activity has many recognized health benefits, the number of inactive persons is increasing worldwide. At the World Health Assembly in May 2018, a resolution to tackle the widespread problem of physical inactivity was made, and in June of the same year, the World Health Organization announced its Global Action Plan on Physical Activity 2018–2030 (GAPPA). To reduce physical inactivity and create a healthy and sustainable world, GAPPA set out the following four strategic objectives: "create active societies," "create active environments," "create active people," and "create active systems." These objectives are achievable through 20 policy actions, each comprising 4–6 items. These strategic objectives and policy actions do not function independently, but are interrelated and can generate co-benefits for one another through a systems-based approach, achieved through the cooperation of each field. Outcomes relate not only to health but also to many Sustainable Development Goals (SDGs). We share these aims when envisioning a long-term Olympic legacy. In the future, it will be necessary for stakeholders to gather at each level, deeply understand GAPPA objectives, and devise an appropriate system plan for execution, reevaluation, sharing, and scaling-up.

目的:本研究の目的は高齢者におけるポリファーマシー,抗コリン作動薬及び鎮静作用薬の使用による薬剤負荷の影響を調査することである.方法:川崎市在住非介護高齢者コホートThe Kawasaki Wellbeing Projectにて2017年3月から12月までに参加した396名を対象とした.ベースライン時の薬剤情報から薬剤数を算出,抗コリン作動薬及び鎮静作用薬に該当する薬剤から対象者のDrug Burden Index(DBI)を算出し薬剤負荷とした.アウトカム指標であるADL,IADL,MMSE,J-CHS,EQ5D5Lについて多変量回帰分析を行い,使用薬剤数又はDBIとの関連性を検討した.調整には性別,年齢,疾患数,教育歴,飲酒歴,喫煙歴を用いた.結果:解析の対象となった389名において年齢の中央値は86歳,男性は48%にあたる187名であった.ポリファーマシーに該当した対象者は243名(62%)であり,DBI該当薬の使用者は142名(36.5%)となった.各アウトカム指標の結果から本集団は身体機能,QOLが高く,フレイルのリスクの低い集団であることが分かった.使用薬剤数はJ-CHS(β:0.04),EQ5D5L(-0.01)と有意に負の関連を示し,DBIスコアはEQ5D5L(-0.04)と有意に負に関連していた.結論:調査結果から本集団は一般的な高齢者と比較すると身体機能及び認知機能の高い健康な集団であることが示された.しかし,ポリファーマシー及び抗コリン作動薬及び鎮静作用薬による薬剤負荷は高齢者のフレイル,QOLの低下と関連していることが示唆された.今後はより大規模で多角的な調査項目を含めた長期間の観察を行うことが望ましい.(著者抄録)

摂取量をたまごの大きさで回答する「たまごの大きさで数える食事チェックシート」の簡単版を開発した.質問用紙は,両面カラー印刷,仕上がりA4版サイズ,二つ折,全4ページの様式とした.質問用紙は,表紙,回答方法の説明,食事内容回答,生活習慣回答ページの4つの構成とした.食事内容回答ページは,見開きページにして一覧で見ることができるようにした.個人結果票は,カラー印刷,A4版サイズ,1枚とし,食事配膳区分を中心とした結果表示とした.摂取量は,たまごの大きさでどのくらいか回答できるため,摂取重量が不明でも本人が回答しやすいものであること,食事配膳区分でのみ回答をするため,質問の項目が少なくなり,食事に興味があまりない方への食事調査で回答のハードルを下げること,食育イベント等で来場した方へ短時間で結果を配布することが可能になると考えられる.妥当性と再現性は,現在解析中である.(著者抄録)

Healthcare data is rapidly increasing as healthcare smart devices are developed. However, statistical analysis of healthcare big data is difficult due to heterogeneity in data quality and missing data. For such data, data mining methods such as clustering and dimension reduction may be useful as their preliminary analysis. In this paper, we show two illustrative applications; one is a principal component analysis of body composition data and the other is a latent topic analysis of hourly step-count dataset recorded by physical activity monitors. From those analyses, we proposed independent indices of body size and hidden obesity and extracted some diurnal patterns in ambulatory activities.

Effects of polypharmacy and anticholinergic/sedative drugs on the physical/cognitive/mental related outcomes of community-dwelling elderly people: The Kawasaki Wellbeing Project

Aim: To assess the burdens of polypharmacy and the drug burden index in community-dwelling elder people.Methods: The survey was conducted on 396 participants who participated in The Kawasaki Wellbeing Project from March to December 2017. We investigated the associations between the drug burden and the physical/cognitive/mental outcomes. The drug burden was determined by calculating the number of medications and the drug burden index (DBI) based on the use of anticholinergic and sedative drugs. A multivariate regression analysis was conducted for the outcome measures ADL, IADL, MMSE, J-CHS, and EQ5D5L after adjusting for the sex, age, number of diseases, education level, smoking history, and alcohol history.Results: A total of 389 subjects were analyzed, the mean age of the population was 86 years old, and 187 people (48%) were male. Polypharmacy was reported in 243 people (62%), and DBI exposure was reported for 142 people (36.5%). We found that this population was physically healthy, with a median ADL of 100, and had high quality of life, with a median EQ5D5L of 0.895. Polypharmacy was found to be related to the J-CHS (β: 0.04) and EQ5D5L (−0.01). The DBI was also related to the EQ5D5L (−0.04).Conclusion: These results showed that even though this population was healthier than the general Japanese elderly population, the drug burden of polypharmacy and high dosages of anticholinergic/sedative drugs exerted significant negative effects on frailty and the quality of life. Additional research should be conducted to investigate the long-term effects of polypharmacy and anticholinergic/sedative drugs on elderly people.

Physical activity, exercise, and health promotion for cancer survivors

Introduction: Physical activity is known to reduce the risk of cancer morbidity and mortality, and to give the health benefits for cancer survivors. For example, after receiving cancer diagnosis, appropriate physical activity can improve prognosis afterwards. Evidence is available to show the risk of cancer-related mortality falls to 50% in the group that continues exercise after a cancer diagnosis. Previous interventional studies also show improvement in physical fitness, quality of life, mood, reduced anxiety and depression/fatigue, an effect on body image/body composition, and an improvement in biomarkers related to cancer risk and outcome.Current status of physical activities: Based on this evidence, the American Cancer Society/American College of Sports Medicine recommends that cancer survivors engage in 150 minutes per week of moderate-intensity aerobic physical activity (or 75 minutes/week of vigorous-intensity aerobic physical activity) and two strength training sessions per week, as well as decreased sedentary time. However, <30% of persons diagnosed with cancer are reported to meet the recommended levels. To maintain high physical activity in real life settings, a feasible strategy in society should be established.Toward Social Implementation: In the 2017 Clinical Practice Guidelines (Canada), evidence supports the promotion of exercise in a group or supervised settings. For sustainable engagement in physical activity, it is important to be able to exercise under guidance and to promote physical activities in daily life, depending on the situation.

Population-based approaches to promote physical activity: the Fujisawa +10 project

Objective: We introduced the physical activity promotion project "Fujisawa +10" and aimed to provide reference information regarding evaluation methods and research findings translating to health education and promotion.Contents: We conducted a community-wide intervention to promote physical activity based on the Japanese physical activity guideline (Active Guide) in cooperation with Fujisawa city. The intervention's key message was "Plus Ten (+10: be active for 10 more minutes than now). We implemented this project in 4 districts for 2 years from 2013, and evaluated its pre- and post-implementation effects using population-based surveys (Phase 1). The RE-AIM evaluation revealed a low degree of reaching residents and improved active guide knowledge, but unchanged physical activity levels. From 2015, we applied it to all districts, considering the Phase 1 results (Phase 2). We aimed to improve the degree of reach and focused on group exercise interventions intended to spread to residents. Elderly individuals who performed the group exercise showed improved physical fitness one year later and consistent physical activity and cognitive function. Then, the number of participants increased by 21% in a year. We extracted features of rules, roles, and tools as important aspects of smooth group activities, and created manuals. These findings are still utilized in Fujisawa city health promotion.Conclusion: This project involved RE-AIM evaluations and strategy improvement. Our findings were translated into the city's policy and scaled up. In Japan, in addition to increases in the practices, it is necessary to further disseminate and implement evaluation frameworks such as the RE-AIM model.
