Sarcopenia is one of the primary risk factors for various adverse health events in later life. However, its pathophysiology in the very old population remains unclear. Hence, this study aimed to examine whether plasma free amino acids (PFAAs) correlate with major sarcopenic phenotypes (i.e., muscle mass, muscle strength, and physical performance) in community-dwelling adults aged 85–89 years living in Japan. Cross-sectional data from the Kawasaki Aging Well-being Project were used. We included 133 adults aged 85–89 years. In this study, fasting blood was collected to measure 20 plasma PFAAs. Measures for the three major sarcopenic phenotypes included appendicular lean mass assessed by multifrequency bioimpedance, isometric handgrip strength, and gait speed from a 5 m walk at a usual pace. Furthermore, we used phenotype–specific elastic net regression models adjusted for age centered at 85 years, sex, body mass index, education level, smoking status, and drinking habit to identify significant PFAAs for each sarcopenic phenotype. Higher histidine and lower alanine levels were associated with poor gait speed, but no PFAAs correlated with muscle strength or mass. In conclusion, PFAAs such as plasma histidine and alanine are novel blood biomarkers associated with physical performance in community-dwelling adults aged 85 years or older.

目的:85-89歳の地域在住高齢者の座位行動を客観的に評価し,テレビ視聴時間を含む関連要因について多面的に検討する。方法:The Kawasaki Aging and Wellbeing Project(KAWP)のベースライン(2017-18年)調査の参加者1,026名に連続7日間の加速度計装着を依頼し,有効データ914名(女性473名)の座位行動を客観的に評価した。重回帰分析を男女別に行い,座位行動との関連要因を3領域(身体状況,社会経済状況,生活習慣)の24因子と年齢,計25因子から検討した。結果:総座位時間・装着時間に占める座位時間の割合は男性1日平均(標準偏差)9.4(1.9)時間・67%,女性8.6(1.8)時間・59%と男性が女性より長く座っており,30分以上継続する座位時間の割合も女性より高かった。重回帰分析により座位行動と関連が認められた要因は,関連が強い順に,男性ではテレビ視聴時間・BMI(正),家事時間・園芸スコア・運動時間・歩行速度・握力・ADL(負),女性ではBMI・テレビ視聴時間(正),家事時間・睡眠時間・運動時間・ADL・外出スコア(負)であった。結論:80歳代後半の地域在住高齢者の座位行動にはテレビ視聴以外に,BMI・歩行速度・握力・ADLなどの身体状況要因,家事・園芸・運動・睡眠・外出などの生活習慣要因が多面的に関連していた。(著者抄録)

断熱改修が成人における冬季の住宅内座位行動および身体活動に及ぼす影響　スマートウェルネス住宅調査による準実験的研究

目的:断熱改修が冬季の住宅内の座位行動と身体活動に及ぼす影響を検討した。方法:断熱改修に意欲的な成人を募集し,冬季2週間のベースライン調査(改修前)と,その1～4年後の同時期に追跡調査(改修後)を行った。住宅内の座位時間と低強度以上の身体活動量(light-to-vigorous physical activity;LVPA)は,加速度計と外出記録から評価した。断熱改修を行った介入群と断念した対照群に対象者を分け,住宅内の座位時間とLVPAの変化との関連を線形混合モデルで検討した。また,脱衣所室温変化量[低下(-1℃未満),変化なし(-1℃以上+1℃未満),上昇小(+1℃以上+5℃未満),上昇大(+5℃以上)]を説明変数に加えた解析も実施した。結果:1,751名(介入群1,640名,対照群111名)を解析対象とした。脱衣所室温は介入群で有意に上昇したが,群間で調査期間のずれが生じたことなどにより,対照群の室温も外気温の上昇に伴い上昇していた。多変量解析の結果,断熱改修と住宅内の座位時間・LVPAの変化に有意な関連は認められなかった。ただし,脱衣所室温変化量は有意に関連していた。室温が変化しなかった場合に比べて,5℃以上改善した場合は,座位時間が減少し,LVPAは増加したが,室温が低下した場合には,座位時間が増加し,LVPAは減少した。結論:断熱改修は住宅内の座位時間とLVPAの変化に関連していなかった。ただし,脱衣所等の室温改善は,住宅内の座位行動を抑制し,LVPAを増加させる可能性が示唆された。今後も検証が必要である。(著者抄録)

地域住民の自主的な運動・スポーツ中における有害事象の調査　横浜市栄区セーフコミュニティ推進協議会スポーツ安全対策分科会による質問紙調査から

目的:栄区セーフコミュニティの活動の一環として実施されたアンケート調査から,栄区民が自主的に実施している運動・スポーツ中における有害事象(事故・けが)の現状と傾向を捉えることを目的とした。方法:2017年に栄区が自主的に運動・スポーツを実施している者を対象に実施した「スポーツ活動時に発生した事故・けがに関するアンケート(選択式・自由記述)」から得られたデータを量的・質的に解析を行った混合研究である。回答が得られた518件のうち,解析が可能であった473件を解析対象として実施した。結果:過去5年間の活動で有害事象があったのは94件(20%)であった。サッカー,バドミントン,バレーボールの順に報告数が多く,下肢の捻挫・靱帯損傷(26件),下肢の筋・腱損傷(20件),頭部・顔面の打撲(7件)が多く発生していた。自由記述の結果からアキレス腱断裂,膝関節前十字靱帯損傷,頭蓋骨骨折,大腿骨骨折,脳出血が発生していた。年代別では,40～50代の筋・腱損傷(16件),60～70代の転倒(11件)が特徴的であった。結論:地域住民が自主的に実施している運動・スポーツ中において、足関節捻挫などの下肢の傷害や高齢者の転倒といった有害事象が発生していることが明らかとなった。これらに対する予防策,対策を講じる必要性が示唆された。(著者抄録)

Adverse events during community residents' independent physical activity and sports: a questionnaire survey conducted by Safe Community Sports Safety Subcommittee in Sakae ward Yokohama city

Objective: This study aimed to determine trends in adverse events during voluntary physical activity and sports performed by Sakae Ward residents.
 Methods: This mixed study quantitatively and qualitatively analyzed the data obtained from a questionnaire (selective and free text) regarding accidents and injuries during sports activities conducted by Sakae Ward in 2017 for people who voluntarily engaged in sports activities. Of the 518 responses, 473 were included in the analysis.
 Results: There were 94 cases (20%) with adverse events in the last 5 years. Soccer, badminton, and volleyball, in that order, had the most frequently reported adverse events. Sprain/ligament injuries of the lower extremities (26 cases), muscle/tendon injuries of the lower extremities (20 cases), and contusions of the head/face (7 cases) were the most common injuries. The results of free descriptions also confirmed the occurrence of serious injuries such as Achilles tendon rupture, anterior cruciate ligament injury of the knee joint, skull fracture, femur fracture, and cerebral hemorrhage. Muscle and tendon injuries (16 cases) were distinctive among residents in the 50–60 age group, while falls (11 cases) was distinctive in the 60–70 age group.
 Conclusion: The study results show that lower limb injuries such as sprained ankle joints and adverse events such as falls among the elderly occurred during physical activity and sports voluntarily performed by Sakae Ward residents. Therefore, preventive measures and countermeasures should be taken to prevent these adverse events.

Objectives: As the world's population is ageing, improving the physical performance of the older population is becoming important. Although diets are fundamental to maintaining and improving physical performance, few studies have addressed the role of these factors in adults aged ≥85 years, and none have been conducted in Asia. This study aimed to determine the dietary patterns and examine their relationship with physical performance in this population. Design: This cross-sectional study (Kawasaki Aging and Wellbeing Project) estimated food consumption using a brief-type self-administered diet history questionnaire. The results were adjusted for energy after aggregating into 33 groups, excluding possible over- or underestimation. Principal component analysis was used to identify dietary patterns, and outcomes included hand grip strength, timed up-and-go test, and usual walking speed. Setting: This study was set throughout several hospitals in Kawasaki city. Participants: In total, 1,026 community-dwelling older adults (85-89 years) were enrolled. Results: Data of 1,000 participants (median age: 86.9 years, men: 49.9 %) were included in the analysis. Three major dietary patterns (DP1: various foods, DP2: red meats and coffee, DP3: bread and processed meats) were identified. The results of multiple regression analysis showed that the score of DP2 was negatively associated with hand grip strength (B, 95% confidence interval: -0.35, -0.64 to -0.06). Conclusions: This study suggests a negative association between hand grip strength and dietary patterns characterised by red meats and coffee in older adults aged ≥85 years in Japan.

Objective: The purpose of this cross-sectional study was to evaluate the sedentary behavior of community-dwelling older adults aged 85–89 years using an accelerometer, and to investigate its correlates, including TV viewing time, from a multifaceted perspective.
 Methods: We asked 1,026 participants in the 2017–2018 baseline survey of “The Kawasaki Aging and Wellbeing Project” to wear an accelerometer for 7 consecutive days. Of these, we evaluated sedentary behavior of valid data from 914 people (473 women). Using multiple regression analysis by gender, we investigated the correlation of 25 factors with sedentary behavior ─24 factors in 3 domains (physical condition, socio-economic condition, and lifestyle) and age.
 Results: The average (SD) total sedentary time and the proportion of sedentary time to wearing time of men and women were 9.4 (1.9) hours/67% per day and 8.6 (1.8) hours/59% per day, respectively. Moreover, men’s proportion of prolonged sedentary time (≥ 30 minutes) was higher than that of women. For men, the following factors were correlated with sedentary behavior in order of strength of association: screen time and BMI (positive); household work time, gardening score, exercise time, walking speed, grip strength, and ADL (negative). For women, the correlated factors were BMI and screen time (positive); household work time, sleep time, exercise time, ADL, and outing score (negative).
 Conclusion: The sedentary behavior of community-dwelling older adults in their late 80s was correlated not only to screen time but also to physical condition factors (such as BMI, walking speed, grip strength, and ADL), and lifestyle factors (such as household work, gardening, exercise, sleep, and going out).

Effect of insulation retrofitting on sedentary behavior and physical activity at home among adults in winter: a quasi-experimental study of the Nationwide Smart Wellness Housing Survey in Japan

Objective: We aimed to examine the effects of insulation retrofitting on sedentary behavior time (SBT) and light-to-vigorous physical activity (LVPA) at home in adults during winter.
 Methods: Adults intending to retrofit their insulation participated in a 14-day baseline survey in winter and a follow-up survey 1–4 years after retrofitting. Participants were classified into an intervention (insulation retrofitting) or a control (non-insulation retrofitting) group. SBT and LVPA changes were calculated using an accelerometer and questionnaires. A multilevel model was fitted to estimate the effects of insulation retrofitting on these changes. As a secondary analysis, the change in dressing room temperature (Td) from baseline was added as an independent variable in the model, categorized as a decrease in Td (<-1.0°C), no change in Td (-1.0°C to +0.9°C), a small increase in Td (+1.0°C to +4.9°C), and a large increase in Td (≥+5.0°C).
 Results: Of the 1751 participants, 1640 were in the intervention group. Both groups experienced an increase in Td. The intervention group experienced a slight increase in outside temperature, whereas the control group experienced a significant increase in outside temperature. Insulation retrofitting was not significantly associated with SBT and LVPA at home; however, the change in Td was significantly associated with SBT and LVPA. Compared to no change in Td, the large increase in Td was associated with a decrease in SBT and an increase in LVPA. In contrast, the decrease in Td was associated with an increase in SBT and a decrease in LVPA.
 Conclusion: Insulation retrofitting was not associated with changes in SBT and LVPA in adults during winter. However, improving room temperature, including Td, could potentially decrease SBT and increase LVPA in adults. Further research is needed to confirm the effect of insulation retrofitting on SBT and LVPA.
