身体活動量と癌(または癌死亡)のリスクの関連については,ここ30年ほどの間に多くの疫学研究(おもに観察研究:症例対照研究およびコホート研究)がなされてきた.この関連の程度は癌の部位により異なり,一部の癌に関しては,疫学研究の関連の一貫性,関連の強さ,容量・反応関係および考えられる機序をもとにエビデンスが確立されてきている.結腸癌および乳癌(女性)では身体活動量との関連は確実で,活動量がもっとも多い群はもっとも少ない群と比較し,前者で30〜40%,後者で20〜30%のリスク減を認めている.前立腺癌では確定的ではないが,10%程度のリスク減を認めている.肺癌,子宮内膜癌については検討数が少ないが,リスク減の可能性がある.その他の癌については研究報告が少ない.癌の一次予防を目的とする身体活動量の目安として大人では中等度の身体活動を30分以上,週に5日以上行うことが推奨されている(著者抄録)

BACKGROUND: Physical activity is associated with health-related quality of life (HRQOL) in the general population, but the effects may be different between sexes. In addition, the effects on resource utilization are not well elucidated. METHODS: A population-based cross-sectional survey was conducted in a rural town, and follow-up data were obtained after 1 year. Physical activity was measured in two ways: amount and maximum intensity. The association between physical activity and the MOS Short Form-36 (SF-36) at baseline, and hospitalization, sick days, and medications in the following year were assessed by multiple regression models. RESULTS: Among 4018 adult residents eligible, 3529 (62.4%) had completed the follow-up questionnaire. A greater amount of exercise was positively correlated with all domains of the SF-36: 2.4-9.5 increase in 100-scale at baseline, but association between maximum intensity and SF-36 was observed in only women. It was also associated with a 20-50% higher utilization of over-the-counter drugs in the following year in both sexes, but the effects of maximum intensity on sick days had different directions for men (relative risk, 2.0-2.4) and women (relative risk, 0.3-0.5). CONCLUSION: The amount of physical activity had positive effects on HRQOL for both men and women, but women had more preferable effects of maximum intensity on HRQOL and resource utilization than men.

Weight change and risk of developing type 2 diabetes

OBJECTIVE: To examine the association between weight change and risk of type 2 diabetes and whether initial weight modifies the association. RESEARCH METHODS AND PROCEDURES: This is a prospective cohort study of 20,187 alumni from Harvard University and the University of Pennsylvania. At baseline in 1962 or 1966, men (mean age, 45.9 years) reported their weight, height, and other risk factors. They also had had their weight and height measured at university entry (mean age, 18.5 years). Participants were followed from baseline to 1998 for type 2 diabetes. RESULTS: During follow-up, 1223 men developed type 2 diabetes. Weight gain significantly increased the risk of this disease. The multivariate relative risks associated with BMI change from university entry to baseline of <-0.5, +/-0.5, >0.5 to 1.0, >1.0 to 1.5, >1.5 to 2.0, >2.0 to 3.0, and >3.0 kg/m(2) per decade were 0.88, 1.00 (referent), 1.29, 2.09, 2.69, 4.67, and 7.02, respectively (p for trend < 0.0001). Even among men with a low initial BMI < 21 kg/m(2), weight gain significantly increased risk; the corresponding relative risks were (no cases), 1.00 (referent), 1.00, 1.93, 2.47, 4.82, and 7.68, respectively (p for trend < 0.0001). DISCUSSION: A low initial BMI does not ameliorate the increase in risk of type 2 diabetes with weight gain. Avoidance of weight gain, even among lean individuals, is important to reduce the risk of this disease.

若年成人肥満者におけるMetabolic Fitnessと減量に伴う改善効果

成人肥満者(18〜39歳)についてmetabolicなリスクを評価し,年齢やBMI,性差の影響,ピーク酸素摂取量の関与,さらに食事プラス運動療法による生活習慣改善による減量が及ぼす改善効果を検討した.肥満者550例を対象とした.若年成人肥満者では,高インスリン血症を特徴的に認め,耐糖能異常よりも早期に脂質代謝異常,高血圧症が発現した.BMIが5増加,または年齢が10歳上昇すると,metabolic syndromeスコアはそれぞれ0.6および0.4増加した.metabolic syndromeの頻度は,BMI 25〜29.9では年齢とともに増加したが,BMI≧30では20代後半から高く,男性70%台,女性50%台であった.Peak V02/kgは,体重とは独立してmetabolic syndromeスコアの減少と関連していた.生活習慣修正による減量の程度に応じて,各種代謝指標が改善した.10%程度の軽度の減量でもMSの頻度は52から21%へと大きく減少した
