構造方程式モデリングを用いて、大学女子アスリートにおけるストレス反応、コーピングおよび生活習慣と月経前症候群(PMS)との関連性について検討した。A大学の運動部30部に所属する女子選手257名より、PMS/PMDD(月経前不快気分障害)評価尺度、ストレス反応尺度、コーピング尺度、食品群別摂取量、運動量、睡眠時間などに関する回答が得られた。PMDDは3.1%、中等度PMSは11.3%にみられ、モデルの検証によってPMS得点に関連している直接的な要因はストレス反応であり、情動焦点型コーピング、食事摂取量はストレス反応を介してPMS得点に関連していることが判明した。ストレッサーの回避や情動焦点型コーピングの強化、偏りのない十分量の食事摂取、運動量の適正化や睡眠によってストレス反応を軽減させることが、PMSの対策として有効である可能性が示唆された。

Evidence on the effects of a community-wide intervention (CWI) on population-level physical activity (PA), especially in the long term, is limited. Therefore, we evaluated the five-year effect of CWI on promoting PA through information dissemination, education, and community support primarily targeting older adults, by incorporating Japanese guidelines, in Fujisawa City, from 2013. To assess the effect of the whole-city intervention, we distributed questionnaires in 2013, 2015, and 2018 to three independent random samples of 3,000 community-dwelling adults (aged ≥ 20 years) using a quasi-experimental study design. Three separate samples responded to the survey (41% at baseline, 46% at the two-year mark, and 48% at the five-year follow-up). The primary outcome was change in PA participation. At the five-year follow-up, PA (median: 120 minutes/day) was significantly higher than at baseline (86 minutes/day) and the two-year follow-up (90 minutes/day). The results of the multivariate analysis indicated that PA among older adults—the primary target population of the CWI—increased significantly at the five-year follow-up, compared to those aged 20–64 (mean difference of change between groups: 14.7 minutes/day, P= 0.029). Among older adults, PA was significantly lower in those with poorer perceived economic status than in their more well-off counterparts at the two-year follow-up (P= 0.003); however, there was no significant difference at the five-year follow-up (P= 1.000). There was a positive interaction between group and period (mean difference of change between groups: 40.9 minutes/day, P= 0.001). In conclusion, the five-year CWI targeting older adults, incorporating national guidelines, improved population-level PA.

Latent Dirichlet allocation (LDA) is a popular topic model for extracting common patterns from discrete datasets. It is extended to the pachinko allocation model (PAM) with a hierarchical topic structure. This paper presents a combination meal allocation (CMA) model, which is a further enhanced topic model from the PAM that has both hierarchical categories and hierarchical topics. We consider count datasets in multiway arrays, i.e., tensors, and introduce a set of topics to each mode of the tensors. The topics in each mode are interpreted as patterns in the topics and categories in the next mode. Despite there being a vast number of combinations in multilevel categories, our model provides simple and interpretable patterns by sharing the topics in each mode. Latent topics and their membership are estimated using Markov chain Monte Carlo (MCMC) methods. We apply the proposed model to step-count data recorded by activity monitors to extract some common activity patterns exhibited by the users. Our model identifies four daily patterns of ambulatory activities (commuting, daytime, nighttime, and early-bird activities) as sub-topics, and six weekly activity patterns as super-topics. We also investigate how the amount of activity in each pattern dynamically affects body weight changes.

成人における冬季の住宅内の暖房使用と座位行動および身体活動：スマートウェルネス住宅調査による横断研究

<p><tt><b>目的</b>：成人を対象に，冬季の暖房使用と住宅内の座位行動および身体活動の関連を検討すること。</tt></p><p><tt><b>方法</b>：本研究は横断研究である。調査は沖縄県を除く</tt>46<tt>都道府県の工務店等を通して募集した断熱改修工事予定のある住宅に居住する</tt>20<tt>歳以上の成人を対象に</tt>2014<tt>年度から</tt>2017<tt>年度までの冬季(</tt>11<tt>月～</tt>4<tt>月)に実施し，</tt>3,874<tt>名から同意を得た。加速度計は連続</tt>14<tt>日間の装着を依頼した。行動日誌から覚醒在宅時間帯を特定し，住宅内の座位行動と身体活動を評価した。コタツおよび脱衣所暖房使用の有無と住宅内の座位行動，身体活動との関連は目的変数を対数変換したうえで，日レベル(室温，オフセット項として覚醒在宅時加速度計装着時間等)，個人レベル(年齢，</tt>BM<tt>等)，世帯レベル(世帯年収，同居人数等)で調整した線形混合モデル(ランダム切片モデル)を男女別に実施した。非標準化偏回帰係数は指数変換し，目的変数への関連の程度を算出した。</tt></p><p><tt><b>結果</b>：有効データは</tt>3,482<tt>名であった。多変量解析の結果，コタツを使用しない場合は住宅内の座位行動時間が約</tt>2<tt>％短く[男性：</tt>exp<tt>(</tt>B<tt>)</tt>=0.98<tt>，</tt>p=0.022<tt>，女性：</tt>exp<tt>(</tt>B<tt>)</tt>=0.98<tt>，</tt>p=0.020<tt>]，その中断回数は約</tt>10%<tt>多く[男性</tt>：exp<tt>(</tt>B<tt>)</tt>=1.10<tt>，</tt>p=0.001<tt>，女性：</tt>exp<tt>(</tt>B<tt>)</tt>=1.11<tt>，</tt>p<0.001<tt>]，身体活動も</tt>5<tt>～</tt>7<tt>％多かった[男性：</tt>exp<tt>(</tt>B<tt>)</tt>=1.07<tt>，</tt>p=0.005<tt>，女性：</tt>exp<tt>(</tt>B<tt>)</tt>=1.05<tt>，</tt>p=0.012<tt>]。脱衣所を暖房する場合も同様の関連が確認された。</tt></p><p><tt><b>結論</b>：コタツを使用しない人，脱衣所を暖房する人は，住宅内の座位行動時間が短く，かつ頻繁に中断し，身体活動量も多かった。局所暖房を使用せず，非居室でも暖房を行い家を暖かく保つことは，住宅内の座位行動を抑制し身体活動を促進する可能性がある。</tt></p>

<b>目的</b>：慶應義塾大学ラグビー部におけるスポーツ外傷・障害・疾患の実態を記述疫学的に明らかにすること。<br> <b>方法</b>：対象は，2011年4月から2018年12月の期間中に慶應義塾大学ラグビー部に在籍した選手368人とした。期間中に発生した練習・試合から離脱しなければならなかったスポーツ外傷・障害・疾患について，練習日・試合あたりの発生件数，診療科別・疾患分類別発生件数，離脱期間および重症度別発生件数について分析した。なお，診療科別・疾患分類別発生件数では，整形外科，脳神経外科疾患について部位別，発生月別，発生状況別，内科疾患については発生年別，月別に分析した。<br> <b>結果</b>：スポーツ外傷・障害・疾患の発生件数は，8年間の選手1人あたり全体1.67件/人，整形外科1.07件/人，内科0.25件/人，脳神経外科（すべて脳振盪）0.24件/人であった。1練習日あたりの発生件数は0.41件/日，1試合あたりの発生件数は1.18件/試合であった。整形外科疾患は部位では下肢が45%，疾患分類では靱帯損傷が46%と発生割合が高く，内科疾患では感冒40%と胃腸炎35%の発生割合が高く，季節性と年ごとの流行が観察された。<br> <b>結論</b>：整形外科疾患は下肢の靱帯損傷，内科疾患は感冒，胃腸炎が多く，脳神経外科疾患は全例脳振盪であった。プレー時間の確保のためには，これらの現状を踏まえた予防策を講じる必要がある。

2020年11月に国際身体活動健康学会（International Society for Physical Activity and Health: ISPAH）は「身体活動を支える8つの投資（Eight Investments That Work for Physical Activity）」を出版した。これは，2010年に同学会が発表した「身体活動のトロント憲章」と2011年の「非感染性疾患予防：身体活動への有効な投資」のうち，後者を最新化するもので，世界保健機関（World Health Organization: WHO）の「身体活動に関する世界行動計画2018-2030」とともに身体活動促進のガイダンスとして有益である。身体活動の促進は人々の健康増進のみならず，より良い社会の実現，国連が定める「持続可能な開発目標（Sustainable Development Goals: SDGs）」の達成に資するとされている。その戦略としてはシステムズアプローチが重要であり，その考えに基づいて8つの領域にわたる対策を推奨している。8つとは，①「学校ぐるみ」のプログラム（whole-of-school programmes），②アクティブな移動・交通手段（active transport），③アクティブな都市デザイン（active urban design），④保健・医療（healthcare），⑤マスメディアを含む一般社会に向けた啓発（public education, including mass media），⑥みんなのためのスポーツとレクリエーション（sport and recreation for all），⑦職場（workplaces），⑧コミュニティ全体のプログラム（community-wide programmes）である。本稿ではその内容を概説するとともに，英語原本およびその日本語訳を要約し，紹介する。

当初，2020年9月8日から12日まで横浜で開催予定であった2020横浜スポーツ学術会議は，突如とした新型コロナウイルス感染症の世界的な蔓延に伴い，開催形式は対面からオンラインへと変わり，また，予定していたプログラムも変更あるいは中止となり，会期も変わるなど大きな影響を受けた。日本運動疫学会が準備を進めていたセッションも一度はすべて中止となったが，関係者の理解と協力を得て，新たに企画を立案し，公開講座という形でFiona Bull氏(世界保健機関ヘルスプロモーション部局身体活動部門長)と鈴木大地氏(スポーツ庁長官)による講演と対談を2020年9月9日に日本とスイスをリアルタイムにバーチャルでつなぐことで実現することができた。当日は300名以上の参加者があり，事前質問も含めてたくさんの質問をいただいたが，すべての質問に答えることができなかったことや，参加者間のディスカッションができなかったこともあり，翌日にオンラインで緊急討論会を開催した。本稿では，運動疫学研究の普及・促進活動の実践報告として，これらの開催までの経緯等を概観したうえで，同講座から得られた知見等を基に，今後の展望について述べる。

<p>社会情勢の変化により、児童の学力育成が重要視されている近年、児童が過ごす教室の温熱・空気環境が、児童の学習効率に影響することに加えて、身体活動が学習効率に与える影響について様々な報告がなされており、身体活動促進が児童・生徒の学力向上に寄与する可能性が示されている。そこで本研究では、教室の温熱・空気環境に加えて、児童の日常的な身体活動量が総合的に児童の学習効率に及ぼす影響を、計算作業成績を用いて客観的に評価することを目的とした。</p>
