症例は27歳、男性。8年前から検診で胸部異常陰影を指摘されていた。2006年3月より咳嗽が出現し精査したところ、胸部CT検査にて右鎖骨頭から発生し縦隔に向かって発育する長径7cm大の腫瘤を認めた。経皮的生検の結果は骨軟骨腫であったが、画像所見から一部肉腫化している可能性も否定できなかったため、腫瘍全摘出術を試行した。病理組織学的診断は軟骨肉腫grade1であった。鎖骨から発生し縦隔へ発育を示す軟骨肉腫は稀であり、文献的考察を含め報告する。(著者抄録)

組織学的に胸腺腫又は胸腺癌と診断された131例の治療成績を検討した。その結果、胸腺癌は7例で、拡大腺全摘術を65例に施行し、内51例には合併切除を行った。重症筋無力症の合併を43例に認めた。予後は、正岡分類ではIVa期及び胸腺癌が、WHO分類ではType Cが有意に不良であった。
