Objective: The Japan Leksell Gamma Knife (JLGK) Society has conducted a prospective multi-institute study (JLGK0901, UNIN000001812) for selected patients in order to prove the effectiveness of stereotactic radiosurgery (SRS) alone using the gamma knife (GK) for 1-10 brain lesions. Herein, we verify the validity of 5 major patient selection criteria for the JLGK0901 trial. Materials and Methods: Between 1998 and 2010, 2246 consecutive cases with 10352 brain metastases treated with GK were analyzed to determine the validity of the following 5 major JLGK0901 criteria; 1) 1-10 brain lesions, 2) less than 10 cm(3) volume of the largest tumor, 3) no more than 15 cm(3) total tumor volume, 4) no cerebrospinal fluid (CSF) dissemination, 5) Karnofsky performance status (KPS) score >/=70. Results: For cases with >10 brain metastases, salvage treatments for new lesions were needed more frequently. The tumor control rate for lesions larger than 10 cm(3) was significantly lower than that of tumors <10 cm(3). Overall, neurological and qualitative survivals (OS, NS, QS) of cases with >15 cm(3) total tumor volume or positive magnetic resonance imaging findings of CSF were significantly poorer. Outcomes in cases with KPS <70 were significantly poorer in terms of OS. Conclusion: Our retrospective results of 2246 GK-treated cases verified the validity of the 5 major JLGK0901 criteria. The inclusion criteria for the JLGK0901 study are appearently good indications for SRS.

The aim of this study was to clarify the reproducibility of the Japanese Ministry of Health, Labor and Welfare (JMHLW) type classification for osteonecrosis of the femoral head. We performed inter-observer and intra-observer trials using 40 sets of magnetic resonance imagings, 20 of which were produced by a 0.5 Tesla (T) superconductive unit and the other 20 produced by a 1.5 T unit, in patients with non-collapsed and asymptomatic osteonecrosis of the femoral head (JMHLW stage 1 or 2). The JMHLW type classification (A, B, C1, or C2) was determined from T1-weighted coronal images at the center of the femoral head. Six orthopedic surgeons independently assessed all 40 images twice, with an interval of 4-5 weeks between sessions. Regarding inter-observer reliability, the percent agreement was 85% and weighted kappa was 0.709 for 0.5 T, versus a percent agreement of 82% and weighted kappa of 0.724 for 1.5 T. Regarding intra-observer reliability, the percent agreement was 82% and weighted kappa was 0.780 for 0.5 T versus a percent agreement of 80% and weighted kappa of 0.800 for 1.5 T. Inter-observer and intra-observer reliabilities did not differ significantly between the 0.5 and 1.5 T units. The JMHLW type classification provided high inter-observer and intra-observer reliabilities.

Genomic DNA is exposed to a constant insult from sources that are both endogenous and exogenous to the cell. REV1 protein, a eukaryotic member of Y family of DNA polymerases, is involved in the tolerance to such DNA damages by providing a translesion DNA synthesis. Here, we examined possible association between REV1 polymorphisms and breast cancer risk in Thai women. Five single nucleotide polymorphisms (SNPs) REV1 (rs3792152, rs3087395, rs3792137, rs959929, rs3087386) were genotyped. The study population consists of 570 patients with histopathologically confirmed breast cancer and 497 controls. Association of genotypes with breast cancer risk was evaluated using multivariate logistic regression to estimate odds ratios (OR) and their 95z confidence intervals (95zCI). Our results revealed that the heterozygote carrier of REV1 SNP (rs3087395) was associated with breast cancer risk with OR=1.59 (95 zCI: 1.11-2.29). In addition, an increased risk was observed for homozygotes at the REV1 SNP (rs 959929) with the OR=1.52 (95zCI: 1.01-2.29). A stratified analysis suggested that the association between the REV1 SNPs and breast cancer is different by the menopausal status. The present results suggest thatREV1 polymorphisms are likely to play a modifying role in the individual susceptibility to breast cancer among Thai women. © The Japanese Environmental Mutagen Society.

Interindividual variation in drug response among patients is well known and poses a serious problem in medicine. The variation could be due to multiple factors including those of the host (e. g., age, genetic and environmental factors) and disease (pathophysiological phenotypes). The complex interplay of these factors may influence pharmacodynamic responses such as adverse effects and efficacy, as well as pharmacokinetic manifestations. However, at present, few biomarkers can predict responders and non-responders, and those who might experience adverse reactions for the same medication and dose. In order to realize personalized medicine, rigorous development of biomarkers is required to identify new biomarkers that may change clinical practice. In this review, we first introduce how biomarkers may be used as targets and the difference between prognostic and predictive markers. Next, we focus on confirmatory clinical trial designs for biomarker development and introduce their application in actual studies. Finally, we introduce exploratory statistical screening techniques for detecting biomarkers in phase I and pharmacokinetics studies.
