Background: Microsurgery requires high skills for suturing using fragile threads, often within narrow surgical fields. Precise tension is required for good healing and to avoid the risk of thread breakage.

Methods: To meet the demands, we developed a novel assist robot utilizing high-precision sensorless haptic technology. The robot adopts a cable-driven mechanism to maintain a distance from the surgical area and enhances compatibility with surgical equipment such as microscopes. The robot performance was verified through in vitro and in vivo experiments using a rat model.

Results: The realization of precise tension control was confirmed in both experiments. In particular, in the in vivo experiments, the developed robot succeeded to produce a knot with an accurate tension of 0.66% error.

Conclusions: The developed robot can realize to control traction force precisely. This technology might open up the window for a full assist robot for microsurgery with haptic feeling.

Influence of prostate cancer status on the prevalence of medication-related osteonecrosis of the jaw.

Objective: The aim of this study was to evaluate the risk of osteonecrosis of the jaw (ONJ) in patients with prostate cancer, particularly the relationship between prostate cancer progression and ONJ development.

Study design: This single-center, retrospective, observational study included 113 patients who received zoledronic acid or denosumab for prostate cancer with bone metastasis between January 2012 and March 2020. The risk of ONJ was evaluated regarding age; antiresorptive drugs; duration of antiresorptive treatment; prostate cancer status, including castration-resistant prostate cancer (CRPC) and prostate-specific antigen level; chemotherapy; radium-223 treatment; corticosteroid treatment; diabetes mellitus; and dental extractions.

Results: Overall, 28 patients had ONJ; 10 patients received zoledronic acid and 18 patients received denosumab. Multiple logistic regression analysis demonstrated that CRPC (odds ratio = 6.01; 95% confidence interval, 1.76-20.05; P = .004) and dental extractions (odds ratio = 12.40; 95% confidence interval, 3.42-44.70; P < .001) were significantly associated with ONJ. In addition, antiresorptive treatment lasting more than 1 year partially mediated between CRPC and development of ONJ.

Conclusion: CRPC and dental extraction are risk factors for developing ONJ, and antiresorptive treatment lasting more than 1 year is a partial mediator between CRPC and ONJ.

Introduction
Osteonecrosis of the jaw (ONJ) occurring after invasive dental treatment often adversely affects patients’ activities of daily living. Long-term administration of strong anti-bone resorptive agents such as bisphosphonates prior to invasive dental treatment is considered an ONJ risk factor; however, pathological mechanisms underlying ONJ development remain unclear.

Materials and Methods
We developed an ONJ mouse model in which a tooth is extracted during treatment with the bisphosphonate zoledronate.

Results
We observed induction of apoptosis in osteocytes, resulting in formation of empty lacunae in jaw bones at sites of tooth extraction but not in other bones of the same mice. We also observed elevated levels of inflammatory cytokines such as TNFα, IL-6 and IL-1 in jaw bone at the extraction site relative to other sites in zoledronate-treated mice. We also report that treatment in vitro with either zoledronate or an extract from Porphyromonas gingivalis, an oral bacteria, promotes expression of inflammatory cytokines in osteoclast progenitor cells. We demonstrate that gene-targeting of either TNFα, IL-6 or IL-1 or treatment with etanercept, a TNFα inhibitor, or a neutralizing antibody against IL-6 can antagonize ONJ development caused by combined tooth extraction and zoledronate treatment.

Conclusions
Taken together, the cytokine storm induced by invasive dental treatment under bisphosphonate treatment promotes ONJ development due to elevated levels of inflammatory cytokine-producing cells. Our work identifies novel targets potentially useful to prevent ONJ.

© 2019 European Association for Cranio-Maxillo-Facial Surgery Purpose: This study compared the diagnostic accuracy of jaw bone invasion (JBI) of oral cancer observed with three-dimensional (3D) SPECT/CT, CT, and MRI, and evaluated the clinical advantages of 3D SPECT/CT compared to conventional two-dimensional (2D) SPECT/CT. Materials and methods: From April 2014 to January 2018, consecutive 16 oral cancer patients with suspected JBI, who had preoperatively undergone the imaging tests, were retrospectively enrolled. The likelihood of JBI was independently scored by a radiologist and oral surgeon. Using 2D or 3D SPECT/CT images, 20 oral surgeons delineated virtual surgical areas on 3D-printed jaws for 3 cases in which the extent of JBI was fully pathologically confirmed. The surgeons completed questionnaires regarding surgical planning and explanations for patients using Likert scales. Results: JBI was found in 9 patients including 5 (56%) with initial bone invasion. 3D SPECT/CT showed very high negative predictive value (100%) and inter-observer agreement (kappa = 0.917). 3D SPECT/CT was more sensitive than CT and MRI when inconclusive findings for JBI were considered negative. Compared to 2D SPECT/CT, 3D SPECT/CT had greater clinical advantages such as surgical planning and explanation to patients (p < 0.005). Conclusion: 3D SPECT/CT is useful not only for detecting JBI but also for surgical planning.

Acute odontogenic infection combined with crowned dens syndrome: A case report

Background: Calcium pyrophosphate dihydrate crystal deposition disease is a condition in which calcium pyrophosphate dihydrate crystal is deposited in joint cartilage and ligaments. Calcium pyrophosphate dihydrate crystal deposition disease that involves calcification around the odontoid process of the second cervical vertebra is called crowned dens syndrome. Crowned dens syndrome is accompanied by fever in addition to acute and intense neck, posterior head, and temporal pain; thus, distinguishing crowned dens syndrome may be difficult in the presence of odontogenic infection. To the best of our knowledge, this is the first report describing a patient with crowned dens syndrome with coexisting odontogenic infection. Case presentation: A 75-year-old Japanese woman was examined in the Emergency Department of this hospital due to a chief complaint of worsened buccal swelling on the left side. An odontogenic infection was considered, and she underwent her first examination. She presented with a body temperature of 37.4 °C, marked swelling and tenderness of her left lower eyelid through to her left cheek, and pain on the left temporal area. Blood tests revealed a leukocyte count of 6700/μL and a C-reactive protein level of 7.15 mg/dL. There was swelling and pain around the gingiva and acute purulent apical periodontitis of left maxillary second premolar. Cellulitis of the left cheek was diagnosed. After performing drainage of the pus, antibiotic treatment was initiated. Although her clinical symptoms improved, blood tests on day 9 of hospitalization revealed a leukocyte count of 6500/μL and a C-reactive protein level of 25.62 mg/dL, which were indicative of worsening symptoms. Computed tomography was performed to evaluate remote infection and images revealed a calcification around the odontoid process of her second cervical vertebra. When she was referred to the Orthopedic Surgery Department, pseudogout of the cervical spine was diagnosed. Subsequently, oral acetaminophen was initiated, and both her leukocyte count and C-reactive protein improved markedly. Conclusions: In the presence of persistent fever and abnormally high leukocyte and C-reactive protein indicative of an inflammatory reaction, coexistence of pseudogout should be considered. In particular, when symptoms of temporal pain are present, the possibility of pseudogout of the cervical spine must be considered in the differential diagnosis.

The effect of improving oral hygiene through professional oral care to reduce the incidence of pneumonia post-esophagectomy in esophageal cancer

© 2018 by The Keio Journal of Medicine. Radical esophagectomy for thoracic esophageal cancer is invasive and frequently results in postoperative pulmonary complications. Postoperative pneumonia is the most common such complication and affects hospital mortality and survival rates. Oral care has been very effective in reducing pneumonia. In Japan, preoperative professional oral care is highly recommended. However, there are few studies on the effect of preoperative improvements in oral hygiene as a result of intervention on the incidence of postoperative pneumonia. The primary end-point of this retrospective study was the incidence of postoperative pneumonia after radical esophagectomy. The oral health levels of 46 patients were individually categorized, and then patients were grouped according to whether they maintained or improved their oral hygiene. At the first dental examination, oral health levels were classified as good in 22 patients and bad in 24. Of the 46 patients studied, 39 patients maintained or improved their oral hygiene (good control group), whereas 7 showed no improvement (bad control group). Postoperative pneumonia occurred in eight patients: four in the good control group and four in the bad control group. Statistical analysis with postoperative pneumonia as a dependent variable showed a significant effect of oral hygiene improvement on the incidence of pneumonia. Logistic regression analysis with this factor as an independent variable demonstrated that the risk of postoperative pneumonia was reduced in the good control group (OR 0.086, 95% CI 0.014-0.529). Therefore, preoperative professional oral care may improve oral hygiene and oral health, which may in turn reduce the incidence of postoperative pneumonia.

Clinical investigation of α1-antitrypsin and lactoferrin in gingival sulcus fluid from elderly patients—A preliminary report

Objectives: To compare levels of α1-antitrypsin (AT) and lactoferrin (LF) in gingival crevicular fluid (GCF) and on the tongue and determine whether MATATACORO® (MA-T), a stabilized chlorine dioxide solution, affects GCF AT and LF levels. Methodology: The α1-antitrypsin and lactoferrin levels in GCF and on the tongue were measured for 18 elderly patients (mean age: 83 years) and correlations were analyzed. AT and LF levels in GCF were assessed before and 1 hr after treatment with MA-T. Results: The α1-antitrypsin and lactoferrin levels were lower on the tongue than in GCF (median 0.6, 0.4 μg/mL vs median 2.4, 3.6 μg/mL). The respective AT and LF levels in GCF and on the tongue were positively correlated (AT: r =.43, LF: r =.69). GCF AT and LF levels were reduced following MA-T treatment (AT: median 3.6 and 1.9 μg/mL and LF: 2.3 and 0.4 μg/mL pre- and post-treatment, respectively; both P <.01). Conclusion: The α1-antitrypsin and lactoferrin levels in GCF were higher than those on the tongue, but AT and LF were detectable for both sampling types. The reduction in GCF AT and LF levels after MA-T treatment suggests that MA-T could be used to control periodontal inflammation.

A case of spindle cell/pleomorphic lipoma on inferior border of the mandible

Although spindle cell lipomas and pleomorphic lipomas were originally considered different entities, the recent WHO classification defines them as spindle cell/pleomorphic lipoma (SCL/PL). This is because of their similar clinical features and the existence of intermediate histopathological features between them. A 77-year-old male presented to our clinic with a painless swelling on the left inferior border of the mandible. Computed tomography revealed a clearly defined mass in the subcutaneous lesion. With a histopathological diagnosis of myxolipoma, demonstrated by biopsy, excisional surgery was performed under general anesthesia. The histopathology revealed SCL/PL with the presence of spindle cells and mature adipocytes. No tumor recurrence had been noted at two year after surgery.

In the tooth extraction process of such cases, bleeding tendency due to pancytopenia, infection etc., becomes a problem. As such, a cautious approach is required. We have confirmed platelet transfusion refractoriness prior to tooth extraction in a myelodysplastic syndrome patient, and hereby report a case of tooth extraction following human leukocyte antigen (HLA)-compatible concentrated platelet transfusion. The patient was a 55-year-old female who was examined at our department due to a request for the extraction of tooth No. 36. Following consultation at the Hematology Department, platelet transfusion was performed as a pretreatment upon hospitalization. Increased body temperature, chills, shivers, and feelings of discomfort in the pharynx were observed. A close examination was performed due to a lack of increase in platelet count, and revealed platelet transfusion refractoriness caused by anti-HLA antibodies. Therefore, the patient was re-hospitalized and transfused with a platelet concentrate-HLA preparation. One hour later, an increase in platelet count was confirmed and the tooth extraction was performed. There was no evidence of bleeding or infection following extraction, and the prognosis was good.

© 2018, International & American Associations for Dental Research. This study investigated a case of spindle cell carcinoma (SpCC) in tongue pathological lesions. The patient experienced a local recurrence and distant metastasis after surgical intervention. Although standard chemotherapy was administered, a granulomatous mass continued to develop. This aggressive growth led to survival of the tumor. Secondary debulking surgery was performed to improve the patient’s quality of life at the request of the patient. Using a tissue sample derived from the secondary debulking surgery, we performed an analysis of the tumor’s cell surface antigens, differentiation potential, metastatic ability, and inhibition potential by anticancer reagents. In vitro analysis revealed that the cell population grown under adherent culture conditions expressed the mesenchymal stem cell (MSC) markers CD73, CD90, and CD105. The cell line established from this SpCC contained colony-forming unit fibroblasts (CFU-Fs) and exhibited multipotent differentiation into several mesenchymal lineages, including bone, cartilage, and fat. The SpCC cells also displayed vigorous mobilization. These characteristics suggested that they had the differentiation potential of mesenchymal cells, especially MSCs, rather than that of epithelial cells. The surgical specimen analyzed in this study resisted the molecular target reagent cetuximab, which is an epidermal growth factor receptor inhibitor. This clinical insight revealed that chemotherapy-resistant SpCC cells have different characteristics compared to most other cancer cells, which are sensitive to cetuximab. Our cell death assay revealed that SpCC cell death was induced by the anticancer drug imatinib, which is known to inhibit protein tyrosine kinase activity of ABL, platelet-derived growth factor receptor α (PDGFRα), and KIT. Here, we report recurrent SpCC with characteristics of MSCs and potential for treatment with imatinib.

© 2018, © The Author(s) 2018. Behçet’s disease is a systemic disorder of unknown etiology. It involves multiple organ systems and is characterized by recurring episodes of oral ulcers as well as ocular, genital, and skin lesions. Oral ulcers can affect tooth brushing and impair proper oral hygiene. As a result, a dental biofilm accumulates, and the condition of the teeth and periodontal tissue deteriorates. The aim of this case report is to highlight the efficacy of periodontal treatment for patients with Behçet’s disease. A 51-year-old man with Behçet’s disease presented with generalized severe periodontitis. After basic treatment of the periodontal tissues, periodontal surgery was performed at several sites with bony defects. However, the patient developed severe stomatitis in the oral mucosa and gingiva after periodontal surgery. Administration of the antimicrobial agent cefdinir had little effect on recovery; however, subsequent administration of sitafloxacin resulted in significant improvement of the stomatitis. This case demonstrates that periodontal therapy is very useful for alleviating the oral signs and symptoms of Behçet’s disease. Systemic antibiotic treatment with sitafloxacin (but not cefdinir) and mechanical debridement were effective in preventing the recurrence of aphthous ulcer outbreaks after periodontal surgery.

Large odontogenic mixofibroma of the mandible excised without visible scar: A case report

© 2018 Asian AOMS(+) ASOMP(+) JSOP(+) JSOMS(+) JSOM(+) and JAMI Combination of a pre-auricular-temporal approach and zygomatic osteotomy is a useful surgical approach for major temporomandibular joint tumours; however, additional approaches may be required to remove tumours expanding under the basal part of the condyle. We describe a patient with a large odontogenic myxofibroma of the basal part of condyle that was excised by a combination of pre-auricular-temporal approach with resection of the zygomatic arch and transoral approach. A 22-year-old man was referred to our department with a painless swelling in the left preauricular area. There was a palpable mass around his left condyle. Computed tomography revealed an infiltrative mass that had expanded to the vicinity of the impacted third molar root apex. The patient had strong cosmetic requirements, and we therefore adopted a combination of pre-auricular-temporal approach with zygomatic osteotomy and transoral approach for resection of the tumour. There was minimal intraoperative bleeding, and no sensory loss in the area innervated by the facial nerve. Histopathological analysis of the excised tumour revealed odontogenic myxofibroma. The scar was located in an inconspicuous area that could be hidden by his hair and in the oral cavity. There were no signs of recurrence or neoplastic transformation 7 years after surgical intervention.

Abstract
Oral lichen planus is a chronic inflammatory mucocutaneous disease. Topical use of steroids and other immuno-modulating therapies have been tried for this intractable condition. Nowadays, tacrolimus ointment is used more commonly as a choice for treatment. However, a number of discussions have taken place after tacrolimus was reported to be carcinogenic. This report describes a patient who applied tacrolimus ointment to the lower lip after being diagnosed with oral lichen planus in 2008, and whose lesion developed squamous cell carcinoma in 2010. Since the relationship between tacrolimus and cancer development has been reported in only a few cases, including this case report, the clinician must be careful selecting tacrolimus as a second-line treatment for oral lichen planus.

We report the case of a 66-year-old man who had a left forehead mass for 5 years.　Positron emission tomography-computed tomography（PET-CT）was performed in 2013 because he had tongue cancer，and it revealed cutaneous accumulation at the left forehead; however，the standardized uptake value（SUV）was low and passage was observed. On PET-CT performed in 2015，an increase in cutaneous accumulation was noted，and therefore，skin biopsy was performed. Pathological findings indicated accumulation in luminal structures of various sizes and a cribriform structure from the dermis to the galea aponeurotica. The luminal structures included basophilic cells，which contained basophilic material. Additionally，neuronal invasion was noted. Therefore，he was diagnosed with primary cutaneous adenoid cystic carcinoma，and surgery was
performed. No recurrence has been noted for 5 months following surgery. Adenoid cystic carcinoma often shows neuronal invasion and regional relapse. Therefore，extended resection and a long-term follow-up are needed. Even if the SUV is low，it is important to perform dermatological examinations.

A case of carcinoma of the maxillary gingiva with insulin-like growth factor II producing non-islet-cell tumor hypoglycemia

Abstract:Non-islet-cell tumor hypoglycemia (NICTH), a condition characterized by recurrent episodes of hypoglycemia, rarely occurs in association with oral cancer. Here, we report a case of carcinoma of the maxillary gingiva with NICTH caused by paraneoplastic production of insulin-like growth factor II (IGF-II). A 72-year-old man with a painful swelling in the left maxillary region was referred to our hospital. A mass with induration was observed in the buccal and palatal regions from the first premolar to the second molar. The clinical diagnosis was carcinoma of the maxillary gingiva, and a biopsy revealed squamous cell carcinoma. The patient received concurrent radiation and chemotherapy with cisplatin. Three weeks after the completion of treatment, three episodes of hypoglycemia developed despite normal intake of meals. On the basis of the results of endocrinological examinations, typical diseases causing hypoglycemia, such as insulinoma and adrenal insufficiency, were ruled out. Furthermore, decreased IGF-I levels were found, which indirectly revealed an increase in IGF-II levels. On the basis of these findings, we diagnosed this case as carcinoma of the maxillary gingiva with NICTH. Continuous intravenous glucose infusion was needed for 3 months from the diagnosis of NICTH until death. NICTH should be borne in mind as one of the complications that can occur during treatment of oral cancer.

A long-term followed-up case of a developing lingual bone defect associated with the sublingual gland

Abstract:Lingual bone defects are usually asymptomatic local concavities of the lingual cortical bone of the mandible, frequently caused by soft-tissue inclusion. In most cases, the soft tissue is the submandibular gland. Only 7 cases of developing lingual bone defects were previously reported in Japan, but none of these cases involved the sublingual gland. We report a rare case of a developing lingual bone defect associated with the sublingual gland, which we followed up for 21 years. The patient was a 58-year-old man who had a developing lingual bone defect in the anterior right side of the mandible. A panoramic X-ray film showed an oval radiolucent area in the apical region of the lower right canine and first premolar tooth. Moreover, electric pulp testing confirmed that all teeth in the lower-right quadrant were vital. A panoramic X-ray film obtained at a nearby dental clinic demonstrated a radiolucent area 15 years before presentation to our department, and the area showed evidence of gradual expansion. Computed tomography revealed a local concavity in the lingual cortical bone of the right side of the mandible. The concavity measured 13 × 9 mm. The signal intensity of the inner tissue of the bone defect was nearly equivalent to that of the sublingual gland on T1-, T2-weighted magnetic resonance imaging. The suspected clinical diagnosis was a lingual bone defect. We performed an incisional biopsy to rule out a neoplastic lesion, and the histopathological diagnosis was normal sublingual gland tissue. On the basis of these results, the final diagnosis was a developing lingual bone defect associated with the sublingual gland. Two years have elapsed, but there is no swelling or induration in the right sublingual gland. The radiolucent area on the panoramic X-ray film showed decreased radiolucency and increased bone volume. Moreover, computed tomography revealed that the concavity of the bone had shrunk.

Desmoid-type fibromatosis of the head and neck in children: a case report and review of the literature.

Background
Desmoid-type fibromatosis is defined as an intermediate tumor that rarely occurs in the head and neck of children. There is no doubt as to the value of complete surgical excision for desmoid-type fibromatosis. However, in pediatric patients, surgeons may often be concerned about making a wide excision because of the potential for functional morbidity. Some studies have reported a lack of correlation between margin status and recurrence. Therefore, we discussed our findings with a focus on the state of surgical margins.

Case presentation
We report an unusual case of a 9-month-old Japanese girl who prior to presenting at our hospital underwent debulking surgery twice with chemotherapy for desmoid-type fibromatosis of the tongue at another hospital. We performed a partial glossectomy and simultaneous reconstruction with local flap and achieved microscopic complete resection. We also reviewed available literature of pediatric desmoid-type fibromatosis in the head and neck.

Conclusions
We described successful treatment for the refractory case of pediatric desmoid-type fibromatosis. The review results showed that some microscopic incomplete resections of tumors in pediatric patients with desmoid-type fibromatosis tended to be acceptable with surgical treatment.

Auricular malformation treated by placement of an osseointegrated implant-supported epithesis using telenavigation and model simulation

We report a case of auricular malformation treated by placement of an osseointegrated implant-supported epithesis using telenavigation and model simulation. A 66-year-old male suffering from auricular malformation and atresia auris of his right ear since birth visited our department in October 2006. The patient desired placement of an ear epithesis supported by osseointegrated implants. Computed tomographic (CT) scans were obtained for use in the navigation system as a prerequisite for the telenavigation and the operative plan was designed in consultation with an overseas oral and maxillofacial surgeon having rich experience in telenavigation surgery. We also constructed a preoperative simulation model and a mirror model of the unaffected left ear based on laser lithography to produce successful operation and symmetrical form of a symmetrical ear epithesis as contrast with identical to that on the left side. The osseointegrated implant surgery was carried out under telenavigation guidance. As a result, safe and accurate positioning of the implants was achieved and a symmetrical and elaborate epithesis could be fabricated.

A titanium screw-retaining temporary denture on the raw surface muscular flap for the immediate maxillary reconstruction

Objective: In any immediate reconstruction of the maxillofacial defect after total maxillectomy, the defect is often covered with a flap, however, dentures cannot be maintained stably by the flap, because of the high mobility and large amount of displacement of the flap under pressure. Therefore, we developed a method in which a muscle flap is transplanted without creating a skin island in the oral cavity and a denture is immediately fixed with titanium screws, and obtained favorable results. Patients and methods: The subjects were 9 patients who underwent resection for maxillary sinus cancer. In all patients, a free rectus abdominis muscle flap was used for the reconstruction and the raw surface of the muscle tissue was left in the oral cavity without creating a skin island. Dentures that had been made preoperatively were fixed on the remaining palate and alveolus with titanium screws for one month, so that they covered the muscle tissue. Results: Good palatal morphology was formed without hanging down after epithelialization of the surface of the muscle tissue. In comparison with the preoperative levels, the masticatory and conversational functions recovered to 77% and 90%, on average. Conclusions: Our method is considered to be one of the useful reconstruction methods for maxillofacial defects.

Soft tissue attachment is a major concern for the improved design of dental and maxillofacial implants. This study evaluated the efficacy of mesh structures for soft tissue attachment in a rat percutaneous model. Four kinds of implant specimens were prepared-TI implants made of titanium cylinders, HA implants of hydroxyapatite-coated titanium, TI-Mesh implants with a titanium mesh covering a groove machined around a titanium cylinder, and similar HA-Mesh implants with a hydroxyapatite-coated mesh. These specimens were implanted percutaneously into the skin tissue of rats. The detachments of the implants were examined during the experimental period of 4 weeks. Survived implants were subjected to mechanical tests for the attachment strength and histological examinations. TI and HA implants demonstrated 0% of survival rates, while TI-Mesh and HA-Mesh showed significantly higher rates of 93.3% and 100% respectively. The attachment strengths were 159 ± 47 kPa in the TI-Mesh and 135 ± 16 kPa in the HA-Mesh. Histological observations revealed that collagen fibers originating from surrounding subcutaneous tissues were anchored to the mesh structures of the TI- and HA-Mesh implants. The results demonstrated the efficacy of the mesh structures for the attachment of soft connective tissues to implants. © 2012 Wiley Periodicals, Inc.

XCT inhibition depletes CD44v-expressing tumor cells that are resistant to EGFR-targeted therapy in head and neck squamous cell carcinoma

The targeting of antioxidant systems that allow stem-like cancer cells to avoid the adverse consequences of oxidative stress might be expected to improve the efficacy of cancer treatment. Here, we show that head and neck squamous cell carcinoma (HNSCC) cells that express variant isoforms of CD44 (CD44v) rely on the activity of the cystine transporter subunit xCT for control of their redox status. xCT inhibition selectively induces apoptosis in CD44v-expressing tumor cells without affecting CD44v-negative differentiated cells in the same tumor. In contrast to CD44v-expressing undifferentiated cells, CD44v-negative differentiated cells manifest EGF receptor (EGFR) activation and rely on EGFR activity for their survival. Combined treatment with inhibitors of xCT-dependent cystine transport and of EGFR resulted in a synergistic reduction of EGFR-expressing HNSCC tumor growth. Thus, xCT-targeted therapy may deplete CD44v-expressing undifferentiated HNSCC cells and concurrently sensitize the remaining differentiating cells to available treatments including EGFR-targeted therapy. © 2012 AACR.

We examined the transfer of levofloxacin (LVFX) into serum, gingiva, dental follicles, and mandibular bone following oral administration of a single 500 mg dose of LVFX to 10 subjects, comprising 3 males and 7 females. The mean concentrations 2.0 to 4.5 h after administration in serum, gingiva, dental follicles, and mandibular bone were 4.85 ± 0.82 μg/mL, 7.76 ± 3.21, 7.38 ± 2.04 μg/g, and 2.90 ± 1.94, respectively. The mean ratios of concentrations in gingiva/serum, dental follicle/serum, and mandibular bone/serum were 1.58 ± 0.53, 1.54 ± 0.50, and 0.59 ± 0.35, respectively. The mean LVFX concentrations in gingival tissue, dental follicles and mandibular bone exceeded the MIC for 90% of clinically isolated oral strains of streptococci and most of the anaerobic bacteria. These results indicated that LVFX penetrated well into oral and maxillofacial tissue and that LVFX 500 mg q.d. treatment was as effective as, or more effective than, 100 mg t.i.d. for the clinical treatment of oral infections.

Objective: Transoral approach from the anterior direction is necessary if an upper spinal tumor is too big to extirpate via the posterior approach alone. To obtain a good anterior view of the surgical field, the mandible, tongue, and soft palate have to be split, and we developed a criterion to select the surgical strategy. Study Design: We indicated a decision-making process of splitting techniques and indications with actual cases and review of literatures. Result: Tumors were located from the level of C1 to the lower part of C3. Computerized tomography and magnetic resonance imaging were important to decide approaches. The orthopedic surgical team invited oral and maxillofacial surgeons team to assist in all transoral anterior approach cases. Conclusions: Vertical splitting of the mandible, incising the skin of the mental region, and splitting the middle of the tongue were combined according to the position of the spinal tumor. © 2012 Elsevier Inc. Burkitt's lymphoma is a malignant lymphoma of high malignancy grade and accounts for less than 1% of all cases of malignant lymphoma. Patients with malignant lymphoma in the oral and maxillofacial region including Burkitt's lymphoma, frequently present with gingival swelling, or a tumor mass/ulcers as the initial symptoms. We present an extremely rare case with bilateral mental nerve paralysis as the initial symptom without not only regional tumor but also brain and nose-pharyngeal area. This report indicates that the unexplained oral abnormalities such as inferior alveolar nerve hypoesthesia must be considered, potentially prodromal symptoms of Burkitt's lymphoma. © 2010 Asian Association of Oral and Maxillofacial Surgeons.

Collagen fiber anchoring platforms for percutaneous devices

In this study, we investigated the use of perforated sheet platforms to improve the attachment of percutaneous devices to surrounding tissues. The model platform was created by a poly(methyl methacrylate) (PMMA) sheet with a matrix of 200-μm diameter perforations covering the groove (width, 3 mm; depth, 0.5 mm) prepared on the PMMA cylinder (diameter, 10 mm; length, 6 mm). Velour cuffs from peritoneal dialysis catheters were used as controls. Specimens were implanted percutaneously in rats, harvested with the surrounding tissues at 4 weeks after surgery, and subjected to mechanical tests and histological observations. The attachment strength of the experimental specimens to tissue was 138.4 ± 123.6 kPa (n = 5, mean ± SD); although it was greater than the 67.23 ± 45.78 kPa (n = 5) of the controls, no statistical significance was found. Histological observations of the experimental specimens revealed the collagen fibers originating from the surrounding tissues, passing through the perforations of the sheet, merging into the collagen fibers running behind the sheet, and thus anchoring the tissues to the device. In contrast, such anchoring of collagen fibers was not evident in the controls. These results suggest that the proposed perforated sheet structures are effective for soft tissue attachment. Mechanical attachment of soft fibrous tissues to implants by using mesh structures

Objectives: Increase of soft tissue attachment is a major concern for improving the design of novel dental implants. This study evaluated the use of mesh structures placed on the implant surface for the anchorage of fibrous tissues. Material and methods: Specimens were prepared by covering their surfaces with a nylon mesh. The following mesh spacings were used: 30, 46, 100, 200, and 500 μm. The specimens were retrieved along with the surrounding subcutaneous tissues at 4 weeks after their implantation into the dorsal subcutaneous tissues of rats. A tensile test was performed by using a material testing machine, and the load required to tear the tissue from the mesh was measured. Histological sections stained with hematoxylin-eosin or Azan were prepared and examined by performing light microscopy. The amount of fibrous tissue contributing to the mechanical attachment of the fibers to the mesh was qualitatively evaluated by performing image analysis. Results: The attachment strength was the greatest in the 200-μm mesh-spacing group and considerably greater than that in the 30-μm mesh-spacing group. In addition, the effective width of the anchoring fibrous tissues was the greatest in the 200-μm mesh-spacing group. As compared with the other groups, the 200- and 500-μm mesh-spacing groups exhibited a considerably greater width of the anchoring fibrous tissues. Conclusions: The efficiency of mesh structures for the attachment of soft tissues to implants was demonstrated. A mesh spacing of approximately 200 μm was observed to be favorable for the anchorage of fibrous tissues. © 2008 The Authors.

Aim: To evaluate the conservativeness of low dose rate interstitial irradiation (LII) for cancer of the mobile tongue. Patients and Methods: Between 1975 and 2002, 100 consecutive patients (71 men, 29 women) underwent LII as curative treatment. Stages were I/II/III/IV= 16/63/16/4. Seventy-one cases were treated with LII alone and 29 cases treated combined with external irradiation. Median total dose of LII was 70 Gy 17 days. Results: Overall, 5- and 10-year local control and LII-treated patients' survival rates were 93% and 91%, 64% and 57%, respectively. Delayed neck metastases were observed in 21% of initially N0 cases, 56% of which could be salvaged by operation. Early stage and well-differentiated tumors carried better prognoses. Conclusion: LII of cancers of the mobile tongue results in good local control and survival. With careful monitoring of patients to ensure early detection of delayed metastases, LII should allow organ conservation and yield favourable therapeutic results compared with those of surgery.
