The recent appearance of multislice computed tomography (CT) has enabled noninvasive imaging of the coronary artery. Particularly, the appearance of 64-row CT has rapidly promoted its spread into routine medical practice. In this report, progress and current state of coronary CT employing multislice CT are outlined.

<p>Angiomyolipoma with epithelial cysts (AMLEC), which has not only a stromal component but an epithelial component, is a rare cystic variant of AML. We present a case of a man in his forties with microscopic hematuria, who had a multilocular cystic tumor in his left kidney. He underwent laparoscopic partial nephrectomy and the tumor was diagnosed as AMLEC histopathologically. We chiefly report the radiologic features of this tumor.</p>

These practice guidelines are recommended by the Asian Society of Cardiovascular Imaging (ASCI), the sole society in Asia designated for cardiovascular imaging, to provide a framework to healthcare providers for suggested essential elements in cardiac magnetic resonance (CMR) examinations of different disease spectra. The guideline is composed of recommendations on the general technique, acquisition of some basic modules, and protocols on stress tests. The protocols for specific diseases are provided in a table format for quick reference to be easily utilized for everyday clinical CMR. © 2010 The Author(s).

There has been a growing need for standard Asian population guidelines for cardiac CT and cardiac MR due to differences in culture, healthcare system, ethnicity and disease prevalence. The Asian Society of Cardiovascular Imaging, as the only society dedicated to cardiovascular imaging in Asia, formed a cardiac CT and cardiac MR guideline working group in order to help Asian practitioners to establish cardiac CT and cardiac MR services. In this ASCI cardiac MR appropriateness criteria report, 23 Technical Panel members representing various Asian countries were invited to rate 50 indications that can frequently be encountered in clinical practice in Asia. Indications were rated on a scale of 1-9 to be categorized into 'appropriate' (7-9), 'uncertain' (4-6), or 'inappropriate' (1-3). According to median scores of the 23 members, the final ratings for indications were 24 appropriate, 18 uncertain and 8 inappropriate with 22 'highly-agreed' (19 appropriate and 3 inappropriate) indications. This report is expected to have a significant impact on the cardiac MR practices in many Asian countries by promoting the appropriate use of cardiac MR. © 2010 The Author(s).

The use of contrast media for cardiac imaging becomes increasing as the widespread of cardiac CT and cardiac MR. A radiologist needs to carefully consider the indication and the injection protocol of contrast media to be used as well as the possibility of adverse effect. There are several guidelines for contrast media in western countries. However, these are focusing the adverse effect of contrast media. The Asian Society of Cardiovascular Imaging, the only society dedicated to cardiovascular imaging in Asia, formed a Working Group and created a guideline, which summarizes the integrated knowledge of contrast media for cardiac imaging. In cardiac imaging, coronary artery evaluation is feasible by non-contrast MR angiography, which can be an alternative examination in high risk patients for the use of iodine contrast media. Furthermore, the body habitus of Asian patients is usually smaller than that of their western counterparts. This necessitates modifications in the injection protocol and in the formula for calculation of estimated glomerular filtration rate. This guideline provided fundamental information for the use of contrast media for Asian patients in cardiac imaging. © 2010 The Author(s).

In Asia, the healthcare system, populations and patterns of disease differ from Western countries. The current reports on the criteria for cardiac CT scans, provided by Western professional societies, are not appropriate for Asian cultures. The Asian Society of Cardiovascular Imaging, the only society dedicated to cardiovascular imaging in Asia, formed a Working Group and invited 23 Technical Panel members representing a variety of Asian countries to rate the 51 indications for cardiac CT in clinical practice in Asia. The indications were rated as 'appropriate' (7-9), 'uncertain' (4-6), or 'inappropriate' (1-3) on a scale of 1-9. The median score was used for the final result if there was no disagreement. The final ratings for indications were 33 appropriate, 14 uncertain and 4 inappropriate. And 20 of them are highly agreed (19 appropriate and 1 inappropriate). Specifically, the Asian representatives considered cardiac CT as an appropriate modality for Kawasaki disease and congenital heart diseases in follow up and in symptomatic patients. In addition, except for some specified conditions, cardiac CT was considered to be an appropriate modality for one-stop shop ischemic heart disease evaluation due to its general appropriateness in coronary, structure and function evaluation. This report is expected to have a significant impact on the clinical practice, research and reimbursement policy in Asia. © 2010 Springer Science+Business Media, B.V.

Computed tomographic attenuation value of coronary atherosclerotic plaques with different tube voltage: An ex vivo study

Objectives: To compare the diagnostic performance of computed tomographic (CT) attenuation and CT attenuation ratio at different tube voltages for ex vivo plaque characterization. Materials and Methods: Human coronary arteries were obtained at the time of autopsy in 15 subjects. The coronary arteries were serially cut into 5-mm-long segments and scanned ex vivo using 4 sets of tube voltages and tube currents (80 kV, 660 mA; 100 kV, 500 mA; 120 kV, 400 mA; and 140 kV, 340 mA). The CT attenuation value at the center of each plaque was obtained, and the ratio of the CT attenuation value at the 80-kV setting divided by that at the 140-kV setting (Hounsfield ratio [HR], 80:140) was calculated. Separate receiver operating characteristic (ROC) analyses were used to assess the usefulness of the CT attenuation value and the 80:140 HR for the differential diagnosis of lipid-rich plaques from other types of plaques. Results: A total of 93 coronary plaques were detected macroscopically. Histological examination revealed 39 lipid-rich, 24 calcified, and 30 fibrotic plaques. At all the tube voltages, the CT attenuation values of the lipid-rich plaques were lower than those of the calcified plaques, whereas the CT attenuation values of the lipid-rich and fibrotic plaques overlapped. An ROC analysis showed that the area under the curve (AUC) for the differential diagnosis of lipid-rich plaques from fibrotic plaques was 0.813 at 80 kV, 0.772 at 100 kV, 0.682 at 120 kV, or 0.651 at 140 kV. Regarding the 80:140 HR, the AUC was 0.952 (0.029). The AUC was significantly larger at 80 and 100 kV and 80:140 HR compared with the AUC at 120 kV. Conclusions: The diagnostic performance of CT analysis for ex vivo plaque characterization was superior at lower energy settings and using the dual-energy method. Objective: To evaluate wall discontinuity, as observed using 64-slice multidetector-row computed tomography (64-MDCT), as a direct finding (DF) indicating the perforation site in patients with gastrointestinal (GI) tract perforations. Methods: We retrospectively studied 41 consecutive patients presenting with acute abdomen and exhibiting extraluminal air (EA) on 64-MDCT. Three readers evaluated the distribution of EA, extraluminal faeces, dirty mass, dirty fat sign, extraluminal fluid collection and bowel wall thickening (i.e. conventional findings, CFs) as well as DFs. Results: Twenty-two cases were surgically or endoscopically confirmed to have upper GI tract perforations, and 19 had lower GI tract perforations. The DFs correctly identified the sites of perforation in 80.5% of patients when 2-mm-thick imaging slices were used. For the detection of upper GI tract perforations, the sensitivity, specificity and accuracy were 95.5%, 94.7% and 95.1% for the DFs and 50.0%, 100% and 73.2% for the CFs, respectively. Significant differences in sensitivity (p<0.001) and diagnostic accuracy (p<0.05) were observed between the DFs and CFs for upper GI perforations but not for lower GI tract perforations. Conclusion: DFs of the perforation site by using 64-MDCT were more sensitive and accurate than CFs for the detection of upper GI tract perforations. © 2009 European Society of Radiology.

CT angiography (CTA) based on the data acquired by multidetector-row CT (MDCT) is an established, minimally invasive modality for imaging peripheral arteries. CTA has been used to assess peripheral arterial disease before treatment, and it has replaced conventional angiography for the diagnostic evaluation of peripheral arteries. MDCT can optimize both the long scan length and spatial resolution. CTA using MDCT depicts the fine structures of vessels. Recently, automated CTA analysis software has been developed for measurement of the vascular lumen. The software can automatically measure the diameters of short axial sections at the post-processing workstation. Measurement of the vascular lumen is useful in the planning of intravascular treatment for peripheral arterial disease. CTA is also utilized in assessing the intravascular lumen after metallic stent placement.
