2003年〜2013年に診療を受けた直腸肛門奇形症例中、肛門形成終了後5年以上経過し、人工肛門造設状態であった7例(男性4例、女性3例、14〜52歳、中央値29歳)を対象に、後方視的に診療録から情報を収集し検討した。男性ではいずれも高位で直腸膀胱瘻、直腸尿道瘻、直腸肛門無形成、不明が各1例、女性では3例とも直腸総排泄腔瘻であった。肛門形成術を含む直腸や尿道、腟に対する外科手術数は2〜10回認められ、肛門形成術を含め幼少期は他施設で治療を受けていた症例が4例、正確な手術回数は不明な症例が3例あった。全例で肛門形成術後に一度は人工肛門が閉鎖されており、その後、種々の理由により人工肛門再造設していた。

<p>Background We present our attempts at reducing the length of incision in living donor left-side hepatectomy without laparoscopic approach. Methods The chief surgeon initially made a 10-cm upper midline incision and performed all procedures through a minilaparotomy without abdominal wall lifting or pneumoperitoneum. For the procedures in the lateral and deep areas, we effectively applied traction to the wound in multiple directions using a wound retraction system so that the chief surgeon could obtain a good direct view. We also placed a fiberscope on the minilaparotomy so that the assistant surgeons could obtain an additional video view via a monitor. Surgeons lengthened the incision at their own discretion if the initial length was thought to be too short for the donor's safety. Since February 2009, we have employed this operation for 19 living donors (12 lateral segmentectomies and 7 left hepatectomies) and compared parameters between the 19 donors and 34 previous donors who underwent the procedure with standard incision (11 lateral segmentectomies and 23 left hepatectomies). Results The resultant length of incision was significantly reduced in operations with reduced incision length as compared with standard incision. Clinical outcomes such as operation time and length of hospital stay were comparable or significantly reduced with the reduced incision. The resultant incision length remained within 10 and 12 cm in lateral segmentectomy and left hepatectomy cases, respectively, whose body mass index was less than 22. Conclusion It appears to be feasible to reduce the incision length for living donor left-side hepatectomy, especially in nonobese cases. © 2014 by Elsevier Inc. All rights reserved.</p>

Long-term native liver fibrosis in biliary atresia

<p>Background &amp; Aims Although liver fibrosis is an important predictor of outcomes for biliary atresia (BA), postsurgical native liver histology has not been well reported. Here, we retrospectively evaluated postsurgical native liver histology, and developed and assessed a novel scoring system - the BA liver fibrosis (BALF) score for non-invasively predicting liver fibrosis grades. Methods We identified 259 native liver specimens from 91 BA patients. Of these, 180 specimens, obtained from 62 patients aged ≥1 year at examination, were used to develop the BALF scoring system. The BALF score equation was determined according to the prediction of histological fibrosis grades by multivariate ordered logistic regression analysis. The diagnostic powers of the BALF score and several non-invasive markers were assessed by area under the receiver operating characteristic curve (AUROC) analyses. Results Natural logarithms of the serum total bilirubin, γ-glutamyltransferase, and albumin levels, and age were selected as significantly independent variables for the BALF score equation. The BALF score had a good diagnostic power (AUROCs = 0.86-0.94, p &lt;0.001) and good diagnostic accuracy (79.4-93.3%) for each fibrosis grade. The BALF score revealed a strong correlation with fibrosis grade (r = 0.77, p &lt;0.001), and was the preferable non-invasive marker for diagnosing fibrosis grades ≥F2. In a serial liver histology subgroup analysis, 7/15 patients exhibited liver fibrosis improvement with BALF scores being equivalent to histological fibrosis grades of F0-1. Conclusions In postsurgical BA patients aged ≥1 year, the BALF score is a potential non-invasive marker of native liver fibrosis. © 2014 European Association for the Study of the Liver. 【背景】小腸機能不全関連肝機能障害(IFALD)に対してFish oilの有効性が報告されている。【対象・方法】従来の内科的治療に抵抗性な3例の新生児外科疾患に合併したIFALD症例に対してfish oil由来静注用脂肪乳剤(Omegaven)を投与した。【結果】3症例は、(1)空腸閉鎖・胎便性腹膜炎、(2)胎便性腹膜炎、(3)腹壁破裂、と胎児診断されていた。新生児期〜乳児期に、(1)腸吻合術、空腸瘻造設、(2)ドレナージ、回腸瘻造設、(3)サイロ造設、腹壁形成、が行われた。経腸栄養が進まないために中心静脈栄養を行ったが、黄疸が増悪しOmegaven治療を行った。2例で投与開始後速やかに黄疸は改善し経腸栄養も促進して中心静脈栄養から離脱したが、1例は肝硬変が進行し死亡した。【結論】早期に対応した2症例では効果があったと考えるが、対応が遅れた1症例では肝硬変が進行し救命し得なかった。不可逆的な肝障害や全身状態の悪化が起こる前に導入したい治療法と考える。(著者抄録)

<p>Congenital prepubic sinus is a rare congenital anomaly situated in the midline of the lower abdomen. We report a case of congenital prepubic sinus, closely associated with a urachal remnant. Preoperative magnetic resonance imaging showed clearly that the sinus tracked the urachus caudally. This finding supports the theory that the anomaly is caused by abnormal remnant tissue originating from the cloacal membrane, which tracks the allantois duct caudally along with fetal longitudinal growth. © 2012 Springer Japan.</p>

<p>Graft fibrosis is a common finding during protocol biopsy examinations after pediatric liver transplantation. We evaluated the clinical utility of liver stiffness measurements by acoustic radiation force impulse (ARFI) imaging, a novel ultrasound-based elastography method, for assessing graft fibrosis after pediatric living donor liver transplantation (LDLT). We performed 73 liver stiffness measurements by ARFI imaging in 65 pediatric LDLT recipients through the upper midline of the abdomen (midline value) and the right intercostal space (intercostal value) around the time of protocol biopsy examinations. Fifty-nine of these liver stiffness measurements could be compared with histopathological findings. Graft fibrosis was assessed according to the degrees of portal and pericellular fibrosis. Significant fibrosis, which was defined as F2 or worse portal fibrosis and/or moderate or worse pericellular fibrosis, was observed in 14 examinations, which had significantly higher midline (P = 0.005) and intercostal values (P &lt; 0.001) than the others. Liver stiffness measurements by ARFI imaging significantly increased with increases in the portal and pericellular fibrosis grades. For the diagnosis of significant fibrosis, the areas under the receiver operating characteristic curve (AUROCs) were 0.760 (P = 0.005) and 0.849 (P &lt; 0.001) for the midline and intercostal values, respectively. The optimal cutoff values were 1.30 and 1.39 m/second for midline and intercostal values, respectively. Slight but significant elevations were noted in the results of biochemical liver tests: serum levels of γ-glutamyltransferase showed the highest AUROC (0.809, P = 0.001) with an optimal cutoff value of 20 IU/L. In conclusion, liver stiffness measurements by ARFI imaging had good accuracy for diagnosing graft fibrosis after pediatric LDLT. The pericellular pattern of fibrosis was frequently observed after pediatric LDLT, and moderate pericellular fibrosis was detectable by ARFI imaging. 小児固形悪性腫瘍の集学的治療における外科治療の役割につき、自験の神経芽細胞腫症例における解析結果を中心に紹介し、検討した。神経芽細胞腫などの小児固形悪性腫瘍では、小さな原発巣からも腫瘍細胞の血管内へのtranslocationが起こり、原発巣とは独立した進展様式で転移巣が形成される可能性が考えられた。自験の進行神経芽細胞腫症例におけるKaplan-Meyer分析の結果では、末梢血や骨髄に微小転移を有する症例は微小転移のない症例よりも有意に生存率が低かった(33.8% vs.87.5、P<0.05)。微小転移のコントロールの可否は、神経芽細胞腫の治療成績に大きく影響している可能性が考えられる。さらにこれらの微小転移細胞と腫瘍幹細胞の関係を指摘した報告もみられる。加えて神経芽細胞腫に対する術中開窓照射併用手術症例の検討からは、転移のある症例においても局所のコントロールの重要性が示唆された。一方、近年、肝芽腫に対する外科治療の選択肢として肝移植が期待されている。一次的治療としても、再発などに対するsalvage治療としても肝移植の有用性が示唆されており、今後、適応基準や、移植前後の化学療法プロトコールの確立が求められる。これらの新たな外科治療の展開により、現在、頭打ちになっている小児固形悪性腫瘍の治療成績のBreak throughを期待したい。(著者抄録)

集中治療管理を要した縦隔リンパ管腫症の2例

症例1:9歳女児。大量胸水貯留による呼吸不全に対し人工呼吸管理および胸腔ドレナージを行った後にICU入室したが、縦隔気腫、気胸、循環血液量減少ショックを合併した。胸水性状および検査所見から乳糜胸と診断してオクトレオチド投与、絶食、中心静脈栄養を行い、CTで縦隔の肥厚と骨透亮像を認め、開胸生検で病理組織学的に縦隔リンパ管腫と診断した。インターフェロンα-2bおよびプロプラノロール投与、胸膜剥離術、OK-432による胸膜癒着術、ステロイドパルス療法を施行したが、現在まで1日1Lの胸水排液が継続している。症例2:2ヵ月男児。大量胸水貯留による呼吸不全に対し人工呼吸管理および胸腔ドレナージを行い、胸水性状および検査所見から乳糜胸と診断してオクトレオチド投与、食事療法を開始し、CTで縦隔の肥厚を認めリンパ管腫症が疑われた。その後、敗血症性ショックを合併し、確定診断に至っていないものの状態が不安定で開胸生検に耐え得る状態ではなかったことから、診断に先行してプロプラノロール投与を行い、状態改善後にOK-432による胸膜癒着術、胸膜剥離術を施行し、開胸生検で病理組織学的に縦隔リンパ腫症と診断した。以後、胸水排液は激減した。

症例は生来健康な9歳8ヵ月の男児。頻回の嘔吐と間歇的腹痛を主訴に紹介受診し、腸重積症と診断。ただちに超音波併用の注腸整復を行ったが明らかな器質的病変は検出できなかった。整復後も腸重積再発を認め、その都度注腸整復を行った。4回目の整復後に腸管内腔に造影剤を満たした状態でCTを施行したところ回腸内腔に腫瘤性病変を認めたため、器質的な回腸病変による腸重積症と判断し、腹腔鏡補助下に手術を施行した。2ポートで腹腔内を観察したが病変は同定できなかった。臍から小腸を引き出して観察すると、回盲弁から約50cmの回腸に病変を触知したため、同部位を含めた小腸部分切除を施行した。病理検査にて、メッケル憩室を伴わない異所性胃粘膜と診断された。本疾患の術前診断は困難で、初回手術時に器質的病変が同定されずに複数回手術を施行されることが多い。そのため、術中は注意深い観察が重要であり、器質的な腸重積の原因として本疾患も念頭におく必要がある。(著者抄録)

1歳2ヵ月。出生当日に小腸の無ガス、注腸検査でmicro colonを、試験開腹でcaliber changeを認め、空腸瘻造設、中心静脈カテーテル(CVC)挿入となった。病理所見よりtolal intestinal aganglionosisと診断し、転院して腸炎予防のため手術を施行した。Treitz靱帯より50cmの部位から腸管拡張を認め、同部位に空腸瘻を、その口側にチューブ瘻を造設し、回盲部から8cmの回腸は残して小腸を切除した。静脈栄養(PN)はcyclic PNとし、肝機能に注意しながら投与カロリーを漸増しアミノ酸、脂肪を管理した。PNは50kcal/kg/日程度に抑え、成分栄養から経腸栄養を開始し、半消化態栄養に移行していった。CVCトラブルは閉塞1回、感染3回認め、入れ替え、エタノールロック療法で対応した。1歳2ヵ月時には身長体重とも3パーセンタイルに達し、最終的にはPN依存率25%程度、ごく軽度の肝機能障害で在宅中心静脈栄養を導入し、術後210日に退院した。
