    下痢症は発展途上国における小児の主たる死亡原因であるが、その原因としてアメーバ赤痢などの原虫性下痢症が挙げられる。これら原虫性下痢症に対する診断法としてPCR法などの遺伝子増幅法が有用だが、途上国においては一般的ではない。そこで、流行地域である発展途上国で実施可能な、アメーバ感染症に対する簡易遺伝子診断法の開発を目的に検討を行った。アメーバ赤痢の病原微生物であるEntamoeba histolyticaから抽出した遺伝子を標的に、等温遺伝子増幅法であるRecombinase Polymerase Amplification(RPA)法と検出紙を組み合わせて使用する、DNAクロマトグラフィー法の開発を目指した。結果として、E.histolytica 10^-1個/μLまで検出可能な検査法を確立した。また、本法は遺伝子抽出から結果判定まで1時間程度と短時間で実施可能であった。等温増幅法であり、サーマルサイクラー・電気泳動が不要であるため、原虫下痢症流行地域でも使用可能な簡易的な診断法であると考える。(著者抄録)

    © 2019 The Author(s). Background: There are no reports on the prevalence of Chagas disease in Japan. Furthermore, screening programs and access to diagnosis and treatment have not been established. This study aimed to clarify the prevalence of Chagas disease among suspected cases in Japan and provide the reference data required for disease control. Methods: Seventeen patients with suspected Chagas disease in Japan between 2012 and 2017 were included in the study. Patients were diagnosed with Chagas disease based on the two different serological tests for antibodies to Trypanosoma cruzi. Real-Time polymerase chain reaction assay and blood culture techniques were performed to confirm T. cruzi parasitemia. Results: Of the 17 patients, 11 (64.7%) were immigrants from Latin America. Ultimately, 6 patients (35.3%) were diagnosed with Chagas disease. Of these 6 patients, median age was 53.5 years, 5 patients were immigrants from Latin American, and 1 was Japanese who had a congenital infection. T. cruzi parasitemia was confirmed in 4 patients (66.7%), and 5 (83.3%) were in the chronic phase (Chagas cardiomyopathy, 4; megacolon, 1). Two patients (33.3%) commenced benznidazole treatment. Conclusion: Our study showed that some patients of Chagas disease living in Japan are already in the chronic phase at diagnosis because of substantial diagnostic delays. Further epidemiological studies on the prevalence of Chagas disease and systematic screening programs for the Latin American population are needed.

    © 2017 Cutaneous leishmaniasis (CL) is gaining attention as a public health problem. We present two cases of CL imported from Syria and Venezuela in Japan. We diagnosed them as CL non-invasively by the direct boil loop-mediated isothermal amplification method and an innovative sequencing method using the MinION™ sequencer. This report demonstrates that our procedure could be useful for the diagnosis of CL in both clinical and epidemiological settings.

  • 髄液と血清からトキソプラズマ遺伝子を検出し得たAIDSトキソプラズマ脳炎の1例

    33歳女。1週間前から発熱と緩徐に進行する体動困難が出現し、来院前日に意識障害を呈して救急搬送された。頭部MRI検査で多発結節性病変を認め、HIVスクリーニング検査が陽性であったため当院転院となった。血液検査で血清トキソプラズマIgG抗体が陽性を示し、頭部造影MRIでは周囲に浮腫を伴いリング状の造影効果をもつ多発結節性病変を認めた。更に、髄液と血清のトキソプラズマPCR検査でいずれも陽性と判明した。トキソプラズマ脳炎(TE)を強く疑い、入院同日よりSulfamethoxazole-TrimethoprimとClindamycinで治療を開始した。入院1週間目には意識障害は改善傾向となり、入院2週間後の頭部MRIでは病変の縮小と周囲の浮腫の改善を認めた。TEに対して6週間の初期治療を行った後、抗HIV療法を開始し、入院9週間目に後遺症なく自宅退院となった。

    本研究はLAMP法またはRPA法とDNAクロマトグラフィを組み合わせた、非結核性抗酸菌皮膚感染症であるブルーリ潰瘍と、皮膚病変を呈する顧みられない熱帯病を同時に診断するmultiplexな皮膚感染症簡易遺伝子診断法の確立を目的として行われる。
    本法の確立により、誰でも実施可能な、簡便・正確な診断が可能になることで、これらの皮膚感染症に対する疾病対策が進展することが期待される。
