Background: The prevalence of nocturia-related sleep problems is higher in older people. This study investigated whether morning and evening light exposure is associated with the sleep problems related to objectively identified nocturnal urination in community-dwelling older adults. Methods: This cross-sectional study analyzed data collected between 2013 and 2016 for the Nagahama Prospective Cohort for Comprehensive Human Bioscience (the Nagahama Study). The study included 1585 participants aged ≥ 65 years who met the criteria for inclusion in the present analysis. An actigraph was used to measure sleep, nocturnal voiding time, and light exposure. The frequency of nocturnal voiding was derived from self-reported entries in the participants’ sleep diaries. Morning light (ML) exposure was defined as the period between waking and 12:00, while evening light (EL) exposure was defined as the period between 18:00 and bedtime. The first uninterrupted sleep period (FUSP) was calculated as the interval between the sleep onset time and the first nocturnal void. Additionally, the ratio of FUSP to sleep period time (FUSP/SPT) was calculated. Associations between light exposure parameters and the frequency of nocturnal voiding, FUSP, or FUSP/SPT were examined using multivariable linear regression models. Results: Of the 1585 participants, 38.7% were male median age was 70 years. The median nocturnal voiding frequency was 1.0 times (IQR: 0.4, 1.4) and the median FUSP was 4.6 h (IQR: 3.2, 5.9). In multivariable linear regression, log ML exposure was significantly associated with FUSP and FUSP/SPT (B = 0.17, 95% CI: 0.02, 0.32; B = 2.20, 95% CI: 0.26, 4.14, respectively). In contrast, the duration of ML exposure ≥ 1,000 lx, EL exposure, and the duration of EL exposure ≥ 50 lx were not significantly associated with the frequency of nocturnal voiding, FUSP or FUSP/SPT. Conclusions: Greater morning light exposure was associated with a longer FUSP. Morning bright light exposure may therefore help delay nocturia-related sleep interruptions, potentially contributing to improved sleep quality among older people.

Background: The combination of bevacizumab and TAS-102 has emerged as a treatment option for metastatic colorectal cancer (CRC). Bevacizumab is an anti-VEGF-A antibody, and TAS-102 is a cytotoxic nucleotide analog. Recently, fruquintinib, an oral, potent and highly selective inhibitor of all three vascular endothelial growth factor receptors (VEGFRs), has been approved for the treatment of chemorefractory CRC. The purpose of this study was to investigate the combined antitumor activity of fruquintinib and TAS-102 in CRC and gastric cancer (GC) models and to reveal possible mechanisms to explain the combined effects. Methods: In vitro angiogenic and lymphangiogenic activities were studied by tube formation assays using HUVECs and HDLECs, respectively. Protein expression was measured by Western blot analysis. Intratumor drug concentrations were determined by liquid chromatography-tandem mass spectrometry analysis. Antitumor activity was studied in xenograft models. Results: Fruquintinib inhibited tube formation in both HUVECs and HDLECs. Trifluridine, an anti-neoplastic component of TAS-102, potentiated the inhibition of tube formation by fruquintinib in HUVECs but not in HDLECs. Suppression of VEGFR downstream signaling, together with induction of DNA damage response, was observed in HUVECs treated with the fruquintinib plus trifluridine combination. In an HT-29 CRC xenograft model, the intratumor levels of trifluridine and its active metabolites were higher following the fruquintinib plus TAS-102 combination treatment than following TAS-102 monotherapy. Antitumor studies demonstrated that the combination of fruquintinib and TAS-102 suppressed tumor growth more potently than individual agents in HT-29 and COLO-205 CRC xenograft models. The combined effect was comparable to that of bevacizumab plus TAS-102 in CRC models and was also observed in SC-2-JCK and MKN45 GC xenograft models. The addition of fruquintinib did not increase body weight loss caused by TAS-102 alone. Conclusions: The combination of fruquintinib and TAS-102 improved antitumor activity in CRC and GC xenograft models, with in vitro evidence of enhanced anti-angiogenic activity of fruquintinib by trifluridine and ex vivo evidence of an increased amount of intratumor trifluridine with fruquintinib. Our data support testing the combination of fruquintinib and TAS-102 in clinical studies.

    Abstract

Circulating tumor DNA (ctDNA) testing in patients with colorectal cancer (CRC) has demonstrated clinical significance in various contexts, including post-curative resection. Therefore, we developed a predictive model integrating preoperative ctDNA levels with radiographic imaging to assess the risk of postoperative recurrence in patients with CRC. Patients with CRC from the GALAXY study with either newly diagnosed or recurrent, curatively resectable liver or lung metastases were enrolled between May 2020 and December 2022, and radiographic images were collected between February 2023 and January 2025. The ratio of preoperative ctDNA levels to tumor metastasis volume from radiographic imaging, ctDNA levels alone, and tumor volume alone was assessed. Overall, 181 and 48 patients from the GALAXY trial had liver and lung metastases, respectively. Among patients with liver metastases, the median progression-free survival (PFS) was 11.4 and 24 months in the high- and low-risk groups classified by the ctDNA model, respectively. Among patients with lung metastases, the median PFS was 12 months and not reached in the high- and low-risk groups classified by the ctDNA/volume model, respectively. Conclusively, incorporating radiological markers of necrosis and refining the tumor volume estimation may further improve model accuracy in liver metastasis settings.

The prognosis of advanced biliary tract cancer remains poor despite up to 40% of tumors harboring actionable molecular alterations. This international collaborative study from Mayo Clinic, Duke Cancer Institute, National Cancer Center East (Japan), and the SCRUM-Japan GOZILA/MONSTAR-SCREEN projects evaluated clinical outcomes in 1049 patients with advanced BTC based on tumor molecular profiling and matched treatment selection. Patients were categorized into non-actionable, actionable with matched targeted therapy (matched), and actionable without matched therapy (unmatched) groups. Among 358 patients (34.1%) with actionable alterations, 160 (44.7%) received matched therapy. The matched group demonstrated significantly longer median overall survival (23.3 months; 95% CI: 19.5-30.4) compared to unmatched (14.7 months; 95% CI: 12.8-17.9) and non-actionable groups (17.1 months; 95% CI: 15.5-18.7; HR 0.57, 95% CI: 0.46-0.71; p 0.0001). In 700 patients receiving ≥2 lines of therapy, matched therapy remained independently associated with improved survival (HR 0.62; 95% CI 0.43-0.90). Notably, patients with actionable tumors who did not receive matched therapy experienced worse outcomes than those with non-actionable disease. These findings highlight the critical need for universal molecular profiling and equitable access to targeted therapies in advanced BTC.
