Cutaneous metastasis of prostate carcinoma treated with electron radiotherapy

<jats:sec><jats:title>Introduction</jats:title><jats:p>Prostate carcinoma is typically diagnosed and treated, and it rarely manifests as cutaneous metastases. We herein report electron radiotherapy for the treatment of cutaneous metastases causing cellulitis, with a durable clinical response achieved.</jats:p></jats:sec><jats:sec><jats:title>Case presentation</jats:title><jats:p>A 70‐year‐old male patient with scrotal cutaneous metastasis of prostate carcinoma was undergoing treatment with docetaxel chemotherapy due to recurring cellulitis originating from the scrotum, and his treatment was interrupted. We administered electron radiotherapy to the scrotal cutaneous metastasis lesions, as irradiation was difficult, and obtained a good clinical effect. Subsequently, he continued chemotherapy, and the scrotal lesions remained clear and dry with no recurring cellulitis for 1 year.</jats:p></jats:sec><jats:sec><jats:title>Conclusion</jats:title><jats:p>Electron radiotherapy is one of the safe and effective treatment options for controlling cutaneous metastasis of prostate carcinoma.</jats:p></jats:sec>

<jats:sec><jats:title>BACKGROUND</jats:title><jats:p>Curative treatment options for patients with early stage hepatocellular carcinoma (HCC) include resection, liver transplantation, and percutaneous ablation therapy. However, even patients with solitary HCC are not always amenable to these treatments. The authors prospectively investigated the clinical outcomes of patients who received stereotactic body radiotherapy (SBRT) for solitary HCC.</jats:p></jats:sec><jats:sec><jats:title>METHODS</jats:title><jats:p>A phase 2 study involving SBRT and optional transarterial chemoembolization (TACE) was conducted in patients with Child‐Pugh grade A or B and underlying, solitary HCC (greatest tumor dimension, ≤4 cm) who were unsuitable candidates for resection and radiofrequency ablation. The prescription dose was 35 to 40 grays in 5 fractions. The primary endpoint was 3‐year local tumor control.</jats:p></jats:sec><jats:sec><jats:title>RESULTS</jats:title><jats:p>From 2007 to 2012, 101 patients were enrolled, and 90 were evaluable with a median follow‐up of 41.7 months (range, 6.8‐96.2 months). Thirty‐two patients were treatment‐naïve, 20 were treated for newly diagnosed intrahepatic failure, and 38 were treated for residual or recurrent HCC as salvage therapy. Thirty‐two patients did not receive TACE, 48 received insufficient TACE, and 10 attained full lipiodol accumulation. The 3‐year local control rate was 96.3%, the 3‐year liver‐related cause‐specific survival rate was 72.5%, and the overall survival rate was 66.7%. Grade 3 laboratory abnormalities were observed in 6 patients, and 8 patients had Child‐Pugh scores that worsened by 2 points.</jats:p></jats:sec><jats:sec><jats:title>CONCLUSIONS</jats:title><jats:p>SBRT achieved high local control and overall survival with feasible toxicities for patients with solitary HCC, despite rather stringent conditions. SBRT can be effective against solitary HCC in treatment‐naive, intrahepatic failure, residual disease, and recurrent settings, taking advantage of its distinctive characteristics. <jats:bold><jats:italic>Cancer</jats:italic> 2016;122:2041–9</jats:bold>. © <jats:italic>2016 American Cancer Society</jats:italic>.</jats:p></jats:sec>

<jats:title>Abstract</jats:title><jats:sec><jats:title>Background and Aims</jats:title><jats:p>Stereotactic ablative body radiotherapy (<jats:styled-content style="fixed-case">SABR</jats:styled-content>) is a relatively new treatment for hepatocellular carcinoma (<jats:styled-content style="fixed-case">HCC</jats:styled-content>). The outcomes of <jats:styled-content style="fixed-case">SABR</jats:styled-content> for previously untreated solitary <jats:styled-content style="fixed-case">HCC</jats:styled-content> unfit for ablation and surgical resection were evaluated.</jats:p></jats:sec><jats:sec><jats:title>Methods</jats:title><jats:p>Untreated solitary <jats:styled-content style="fixed-case">HCC</jats:styled-content> patients treated with <jats:styled-content style="fixed-case">SABR</jats:styled-content> were retrospectively studied. Between 2005 and 2012, 221 <jats:styled-content style="fixed-case">HCC</jats:styled-content> patients underwent <jats:styled-content style="fixed-case">SABR</jats:styled-content>. Among them, patients with untreated solitary <jats:styled-content style="fixed-case">HCC</jats:styled-content>, treated with only <jats:styled-content style="fixed-case">SABR</jats:styled-content> or <jats:styled-content style="fixed-case">SABR</jats:styled-content> preceded by transarterial chemoembolization, were eligible. Based on baseline liver function and liver volume receiving ≥ 20 <jats:styled-content style="fixed-case">G</jats:styled-content>y, 35–40 <jats:styled-content style="fixed-case">G</jats:styled-content>y in five fractions was prescribed to the planning target volume surface.</jats:p></jats:sec><jats:sec><jats:title>Results</jats:title><jats:p>Sixty‐three patients were eligible, with a median follow‐up duration of 31.1 (range 12.0–88.1) months. No patients were lost to follow‐up. Twenty patients were treated with only <jats:styled-content style="fixed-case">SABR</jats:styled-content>. In 43 patients treated with <jats:styled-content style="fixed-case">SABR</jats:styled-content> preceded by transarterial chemoembolization, accumulation of lipiodol in the tumor remained complete in five, a partial defect in 38 on pre‐<jats:styled-content style="fixed-case">SABR</jats:styled-content> computed tomography. The 1‐, 2‐, and 3‐year local control rates were 100%, 95%, and 92%, respectively; the intrahepatic recurrence‐free rates were 76%, 55%, and 36%, respectively; and the overall survival rates were 100%, 87%, and 73%, respectively. Grade 3 laboratory toxicities in the acute, subacute, and chronic phases were observed in 10, 9, and 13 patients, respectively, and ascites occurred in one patient.</jats:p></jats:sec><jats:sec><jats:title>Conclusions</jats:title><jats:p>Local control and overall survival after <jats:styled-content style="fixed-case">SABR</jats:styled-content> for untreated solitary <jats:styled-content style="fixed-case">HCC</jats:styled-content> were excellent despite the candidates being unfit for resection and ablation. <jats:styled-content style="fixed-case">SABR</jats:styled-content> is safe and might be an alternative to resection and ablation.</jats:p></jats:sec>
