Assessment of adverse events and near-misses during voluntary sports by Japanese middle-aged and older adults: 14-month prospective study

This study aimed to prospectively record the occurrence of adverse events and near-misses during sports in middle-aged and older adults who voluntarily engaged in sports in the community and to clarify the frequency and characteristics of such events. Participants were comprised of 14 groups (14 sports) of middle-aged and older adults who voluntarily played sports in Sakae Ward, Yokohama. The follow-up period was 14 months (November 1, 2022, to December 31, 2023). Representatives of the target groups reported once a month to Sakae Ward via postcards regarding the status of their activities and the occurrence of adverse events and near-misses. The survey items included the age of the main participants in each group, number of activities during the month, duration of each activity, number of participants per activity, adverse events, and near-misses during sports. During the 14-month observation, eight adverse events and two near-misses were reported in the 14 groups, mainly comprising middle-aged and older adults. Of the eight adverse events, four were falls, all occurring in those aged ≥70 years and triggered by environmental problems. In this study, there were only two near-misses compared to eight adverse events, suggesting that the participants may have been unaware of near-misses. Therefore, even for sports played by groups of residents, it is important to identify risks by assessing health conditions prior to exercise and to implement initiatives to foster safety awareness among participants, such as hazard prediction training.

Epidemiology of adverse events related to sports among community people: a scoping review

Objectives Numerous reports have described injuries and illnesses in competitive athletes, but studies on leisure-time physical activity and associated adverse events in the general population have not been adequately reviewed. This study aimed to summarise the previous findings on this topic. Design Scoping review. Data sources PubMed and Ichushi-Web for articles in English and Japanese, respectively (13 April 2023). Eligibility criteria for selecting studies Articles on adverse events related to sports performed by € individuals and groups active in the community' were included, whereas those on elite athletes, exercise therapy and rehabilitation, and school sports were excluded. Terms related to physical activity, exercise, sports and adverse events were used for the search strategies. Results The literature search yielded 67 eligible articles. Most articles were from the USA, Japan and Australia. Running, scuba diving, rugby and soccer were the most commonly reported sports. Adults were the most common age category in the samples. The most commonly reported adverse events were injuries; only 10 articles reported diseases. 13 longitudinal studies reported the frequency of adverse events based on the number of events/participants×exposure. Conclusion Adverse events such as sports trauma, disability and certain diseases occur sometimes during sporting activities by residents; however, the articles identified in this review showed biases related to the countries and regions where they were published and the sports disciplines and types of adverse events reported, and articles reporting the frequency of adverse events were also limited. This highlights the need for more high-quality observational studies on diverse populations in the future.

Assessment of adverse events and near-misses during voluntary community-driven sports activities by community residents: A cross-sectional study

Although physical activities have many health benefits, adverse events and near misses, such as injuries and falls, can occur during these activities. This study aimed to assess the occurrence of adverse events and near misses during sports activities conducted independently by community residents. A survey questionnaire was sent via the internet to the leaders or directors of sports organizations at six public sports centers or associations. In total, 108 individuals answered the survey, with 60% male and 40% female respondents. Individuals aged 50–69 years accounted for 60% of the total number of respondents. All respondents were asked about their experiences of adverse events and near misses within the past three years: the reports of these incidents were obtained using the recall method. Duplicate adverse events and near misses were identified based on the sports discipline, time of occurrence, and sex and age of person involved to determine if there were duplicate reports. Most of the respondents’ activities as staff were performed once a month, with each activity lasting 1–2 h. Forty-five adverse events were reported, including 26 injuries, 13 falls, and 6 others (such as heat stroke, vertigo, and presyncope). Twenty-four near misses were reported, including 12 near collisions with people or objects, five near falls, and seven other incidents. We found that approximately 30% of the respondents experienced adverse events, suggesting the need for documentation of adverse events, implementation of safety measures, and proper safety education for operating staff.

Impact of Housing Conditions on Physical Fitness Changes in College Rugby Players during Team Training Interruptions: A retrospective longitudinal study

The global outbreak of novel coronavirus infection in the winter of 2020 led to city lockdowns and curfews in many parts of the world, forcing university athletes to suspend training. De-training negatively affects athletic performance. In school sports, temporary closures due to infectious diseases often occur yearly. Therefore, this study aimed to clarify the differences in the changes in various physical fitness factors between two different residential environments during the suspension of team activities. The observation period was 7 months, from March to September 2020. The baseline period was defined as the period before the voluntary suspension of team activities (March), the de-training period was the timeframe of suspension of team activities (April-May), and the follow-up period was defined as the period after the resumption of team practice (June-September). The study participants included 17 dormitory students and 14 home-schooled students in a university rugby team. The dormitories were equipped with training equipment and space for minimal training. The team staff provided a training program for both dormitory and non-dormitory students, which could be followed at home during periods when team practices were suspended. This study showed sig-nificant differences between dormitory and home-schooled students in the maximum lifting weight measurement of narrow chinning at baseline and after de-training. Other physical fitness factors did not differ significantly between groups or measurement periods. Although this study sample size was small and sufficient power could not be obtained, home weight training could maintain a certain level of maximum muscle strength as in the present study, suggesting that preparation for back exercises that can be performed at home is essential for the back muscles.

Digital Peer-Supported App Intervention to Promote Physical Activity Among Community-Dwelling Older Adults: Nonrandomized Controlled Trial.

BACKGROUND: The use of mobile apps has promoted physical activity levels. Recently, with an increasing number of older adults accessing the internet, app-based interventions may be feasible in older populations. Peer support-based interventions have become a common method for promoting health-related behavior change. To our knowledge, the feasibility of using digital peer support apps (DPSAs) to increase physical activity among older adults and its impact on physical activity and physical function have not been investigated. OBJECTIVE: This study aims to assess the feasibility of using DPSAs in older adults and to assess changes in physical activity and physical function in DPSA users. METHODS: We conducted a nonrandomized controlled trial of older adults aged ≥65 years. We recruited participants for 2 distinct 12-week programs designed to increase physical activity. Participants could choose between an intervention group (app program and exercise instruction) or a control group (exercise instruction only). DPSA creates a group chat for up to 5 people with a common goal, and participants anonymously post to each other in the group. Once a day, participants posted a set of their step counts, photos, and comments on a group chat box. The intervention group used the DPSA after receiving 2 face-to-face lectures on its use. The participants were characterized using questionnaires, accelerometers, and physical function assessments. The feasibility of the DPSA was assessed using retention and adherence rates. Physical activity was assessed using accelerometers to measure the daily step count, light intensity physical activity, moderate to vigorous intensity physical activity (MVPA), and sedentary behavior. Physical function was assessed using grip strength and the 30-second chair-stand test. RESULTS: The participants in the intervention group were more frequent users of apps, were more familiar with information and communication technology, and had a higher baseline physical activity level. The retention and adherence rates for the DPSA intervention were 88% (36/41) and 87.7%, respectively, indicating good feasibility. Participants in the intervention group increased their step count by at least 1000 steps and their MVPA by at least 10 minutes using the DPSA. There was a significant difference in the interaction between groups and intervention time points in the daily step count and MVPA (step count, P=.04; duration of MVPA, P=.02). The DPSA increased physical activity, especially in older adults with low baseline physical activity levels. CONCLUSIONS: The feasibility of DPSA was found to be good, with the intervention group showing increases in daily steps and MVPA. The effects of DPSA on step count, physical activity, and physical function in older adults with low baseline physical activity should be investigated using randomized controlled trials.

A comparation of injury rates in high school baseball players between the normal year and restricted activity year due to COVID-19

This study compared the injury rate of 117 high school baseball players between the year of 2019 and 2020, in which outside activity was restricted due to COVID-19. The number of injury incidence rates were surveyed for each of the positions for both years. An increase in shoulder and elbow injuries was found during summer, 2020 compared to 2019. The number of ankle and foot injuries also increased during winter, 2020. Also, the number of shoulder and elbow injuries increased in infield players during fall, 2020. The results of this study suggest that continuous injury surveys will be necessary to identify the prevalence of high school baseball injuries for unusual circumstances.

Adverse events during community residentsʼ independent physical activity and sports: a questionnaire survey conducted by Safe Community Sports Safety Subcommittee in Sakae ward Yokohama city

Objective: This study aimed to determine trends in adverse events during voluntary physical activity and sports performed by Sakae ward residents.Methods: This mixed study quantitatively and qualitatively analyzed the data obtained from a questionnaire (selective and free text) regarding accidents and injuries during sports activities conducted by Sakae ward in 2017 for people who voluntarily engaged in sports activities. Of the 518 responses, 473 were included in the analysis.Results: There were 94 cases (20%) with adverse events in the last 5 years. Soccer, badminton, and volleyball, in that order, had the most frequently reported adverse events. Sprain/ligament injuries of the lower extremities (26 cases), muscle/tendon injuries of the lower extremities (20 cases), and contusions of the head/face (7 cases) were the most common injuries. The results of free descriptions also confirmed the occurrence of serious injuries such as Achilles tendon rupture, anterior cruciate ligament injury of the knee joint, skull fracture, femur fracture, and cerebral hemorrhage. Muscle and tendon injuries (16 cases) were distinctive among residents in the 50–60 age group, while falls (11 cases) was distinctive in the 60–70 age group.Conclusion: The study results show that lower limb injuries such as sprained ankle joints and adverse events such as falls among the elderly occurred during physical activity and sports voluntarily performed by Sakae ward residents. Therefore, preventive measures and countermeasures should be taken to prevent these adverse events.

目的:栄区セーフコミュニティの活動の一環として実施されたアンケート調査から,栄区民が自主的に実施している運動・スポーツ中における有害事象(事故・けが)の現状と傾向を捉えることを目的とした。方法:2017年に栄区が自主的に運動・スポーツを実施している者を対象に実施した「スポーツ活動時に発生した事故・けがに関するアンケート(選択式・自由記述)」から得られたデータを量的・質的に解析を行った混合研究である。回答が得られた518件のうち,解析が可能であった473件を解析対象として実施した。結果:過去5年間の活動で有害事象があったのは94件(20%)であった。サッカー,バドミントン,バレーボールの順に報告数が多く,下肢の捻挫・靱帯損傷(26件),下肢の筋・腱損傷(20件),頭部・顔面の打撲(7件)が多く発生していた。自由記述の結果からアキレス腱断裂,膝関節前十字靱帯損傷,頭蓋骨骨折,大腿骨骨折,脳出血が発生していた。年代別では,40～50代の筋・腱損傷(16件),60～70代の転倒(11件)が特徴的であった。結論:地域住民が自主的に実施している運動・スポーツ中において、足関節捻挫などの下肢の傷害や高齢者の転倒といった有害事象が発生していることが明らかとなった。これらに対する予防策,対策を講じる必要性が示唆された。(著者抄録)

Ice slurry ingestion improves physical performance during high-intensity intermittent exercise in a hot environment.

Ice slurry ingestion enhances exercise performance by lowering the core body temperature. However, an operational issue related to this ingestion is the requirement for a high intake of 7.5 g·kg-1 to produce the desired effects. We investigated the effects of the intake of low amounts of ice slurry at -2°C on the tympanic temperature and exercise performance during repeated high-intensity intermittent exercises in a hot environment. This study was a randomized, crossover study, with a 6-day washout period. Twelve university rugby union players performed two 30-min sessions of high-intensity intermittent exercises separated by a 15-min half-time break on a cycle ergometer in a hot environment (28.8°C ± 0.1°C, 49.5% ± 0.6% relative humidity). The participants ingested 450 g of -2°C-ice slurry (ICE), or a 30°C-beverage (CON) having the same composition as ICE, or 30°C-water (WAT) during the half-time break. The tympanic temperature and skin temperature were measured as the physiological data, and the peak power and mean power as the exercise performance data. The tympanic temperature at the half-time break and beginning of the 2nd session was significantly lower in the ICE group as compared with the CON and WAT groups. The skin temperature at the half-time break was significantly lower in the ICE group as compared with the WAT group. While the peak power and mean power during the 2nd session were significantly greater in the ICE group as compared with the CON and WAT groups. Our findings suggest that even the intake of lower amounts, as compared with those used in previous studies, of low-temperature ice slurry can reduce the body temperature and improve the peak power. These results suggest that intake of low-temperature ice slurry as a strategy for internal body cooling is useful for improving endurance exercise performance in hot environments.

学生アスリートにおけるスポーツ外傷・障害受傷後の競技復帰に関する主観的満足度に及ぼす要因　競技からの長期離脱者に注目して

本研究は競技活動からの長期離脱を要した者を対象とし、競技復帰の主観的な満足度に影響を与える要因を検討した。対象は大学及び専門学校1年生とし、調査票の回答率は99.3%(1076名中1069名)であった。調査内容は高校3年間におけるスポーツ外傷・障害による競技休止中の状況に関する質問(19項目)とした。回答結果から、競技復帰を「成功体験」ととらえている者(343名)を分析対象とした。離脱期間の中央値+0.25四分位偏差以上の者を長期離脱者(127名)とし、主観的満足度の高値群と低値群を目的変数、調査票の19項目を説明変数としてロジスティック回帰分析を行った。その結果、「休止期間中の練習の休みやすさ」の回答で有意に関連性がみられ、長期間離脱者における満足度高値群では、適切に休みを取りやすい環境であると捉えており、低値群では休みがとりづらく、無理をしてしまう環境と捉えていることが明らかとなった。(著者抄録)

A comparison of physical characteristics in starters and non-starters in a Japanese university rugby football players

The purpose of this study was to compare the characteristics of starters and nonstarters with respect to the anthropometric and physical fitness in a Japanese university rugby team. Our sample pool consisted of 54 players of the K University rugby team, who were registered for the official games (all 7 games/year) in 2018 and 2019. The starter group (26 players) comprised of players with more than 3 games/year while the non-starter group (28 players) comprised of players that registered for official games but played less than 3 games. Anthropometric parameters, such as height, body mass, skinfold (8 items), girth (5 items), body composition, muscular strength, intermittent endurance, and sprint time, were assessed. We found that the starter group for forwards had lower skinfold (Biceps, Iliac crest, Supraspinale, Abdominal, Front thigh, and Medial calf) and percentage body fat than in the non-starter group. The starter group for backs had muscle strength of narrow chinning is higher than in the non-starter group. Finally, our study helped to clarify the physical factors that influence the difference in athletic performance between starters and non-starters in university rugby team. The forward starters had lower percentage body fat and better body composition and backs starters had better narrow chinning. The findings provide information to university rugby players who want to become starters, as well as help the coaching staff understand the factors that need to be strengthened in the players.

Monitoring of Post-Brain Injuries By Measuring Plasma Levels of Neuron-Derived Extracellular Vesicles

Background:

Extracellular vesicles (EV) released from neurons into the blood can reflect the state of nervous tissue. Measurement of neuron derived EV (NDE) may serve as an indicator of brain injury.

Methods:

A sandwich immunoassay was established to measure plasma NDE using anti-neuron CD171 and anti-EV CD9 ([CD171 + CD9+]). Plasma samples were obtained from commercial sources, cross-country (n = 9), football (n = 22), soccer (n = 19), and rugby (n = 18) athletes over time. Plasma was also collected from patients undergoing total aortic arch replacement (TAR) with selective cerebral perfusion during cardiopulmonary bypass before and after surgery (n = 36).

Results:

The specificity, linearity, and reproducibility of NDE assay (measurement of [CD171 + CD9+]) were confirmed. By scanning electron microscopy and nanoparticle tracking, spherical vesicles ranging in size from 150 to 300 nm were confirmed. Plasma levels of NDE were widely spread over 2 to 3 logs in different individuals with a significant age-dependent decrease. However, NDE were very stable in each individual within a ± 50% change over time (cross-country, football, soccer), whereas rugby players were more variable over 4 years. In patients undergoing TAR, NDE increased rapidly in days post-surgery and were significantly ( P = .0004) higher in those developing postoperative delirium (POD) (n = 13) than non-delirium patients (n = 23).

Conclusions:

The blood test to determine plasma levels of NDE was established by a sandwich immunoassay using 2 antibodies against neuron (CD171) and exosomes (CD9). NDE levels varied widely in different individuals and decreased with age, indicating that NDE levels should be considered as a normalizer of NDE biomarker studies. However, NDE levels were stable over time in each individual, and increased rapidly after TAR with greater increases associated with patients developing POD. This assay may serve as a surrogate for evaluating and monitoring brain injuries.

Screening for Jones Fractures in University Rugby Union Players

背景:大学ラグビー部で実施しているJones骨折検診の結果とJones骨折の発生状況を報告する。方法:2017年、2018年に実施したJones骨折検診に参加した大学ラグビー部員50名を対象とした。検診結果は2017年をS1群、2018年をS2群とした。検討項目は既往歴、練習状況、圧痛、エコー検査、股関節内旋角度とした。また、2017年に実施した検診日を基準として、過去2年間をPre群、検診後2年間をPost群としてJones骨折発生件数を比較した。結果:既往歴、練習状況、圧痛、エコー検査、股関節内旋角度でS1群S2群間に有意差はみられなかった。Jones骨折の発生数はPre群3件、Post群0件であった。考察:検診によって選手やスタッフの意識、行動に変容があったのではないかと推察される。まとめ:検診前後2年間のJones骨折発生数は減少したが、さらなる調査が望まれる結果であった。(著者抄録)

Objective: To clarify the incidence rates of sports injuries, including those due to trauma, overuse, and diseases, a descriptive epidemiological study was conducted in players from the Keio University rugby football club.Methods: The study participants were 368 players who belonged to the Keio University rugby football club from April 2011 through December 2018 (1,049 players year). We analyzed the incidence per practice day and game, by clinical department category and disease classification, and by unplayable period and severity of the sports injury and disease that led to leave from practice and game during the study period. In addition, we analyzed the incidence of sports injuries and diseases by body part affected, month of onset, orthopedic and neurosurgical disease statuses, and year and month of onset of internal diseases according to clinical department and disease classification.Results: The incidence of sports injuries and diseases was 1.67 cases/player overall, 1.07 cases/player in the orthopedic diseases, 0.25 case/player in the internal diseases, and 0.24 case/player in the neurosurgical diseases (all concussions) over 8 years. The incidence rates per practice day and game were 0.41 and 1.18 cases, respectively. The conditions with the highest incidence rates were as follows: orthopedic diseases of the lower limb, 45%; ligament injuries, 46%; common cold, 40% of all internal diseases; and seasonal and annual cases of gastroenteritis, 35%.Conclusion: This study shows that many cases of ligament injuries of the lower limb were treated as orthopedic diseases; and common cold and gastroenteritis, as internal diseases. All cases of concussion were treated as neurosurgical diseases. Given these facts, steps must be taken to secure the duration of a play.
