<h4>Background</h4>Registered dietitians are rarely employed at community pharmacies in Japan, even though dietetic advice might benefit some patients.<h4>Objective</h4>To clarify the present status of dietetic consultation provided by registered dietitians and their collaboration with pharmacists in community pharmacies.<h4>Methods</h4>We conducted a cross-sectional questionnaire-based survey of pharmacists and registered dietitians who work in community pharmacies. The surveyed items were: frequency of dietetic consultation, awareness of one's knowledge and ability to conduct dietetic consultation, concerns, pharmacists' recognition of the need for nutritional support at community pharmacies, and cooperation between registered dietitians and pharmacists.<h4>Results</h4>Sixty-six registered dietitians, 53 pharmacists in pharmacies with registered dietitians/dietitians, and 110 pharmacists in pharmacies without registered dietitians/dietitians responded. The frequency of dietetic consultation regarding obesity and hypertension was significantly higher for registered dietitians than for pharmacists. The ability to conduct dietetic consultation regarding diseases/conditions such as kidney disease not requiring dialysis, hyperuricemia, gout, obesity and hypertension was also significantly higher for dietitians than pharmacists. More than 70% of pharmacists recognized the importance of nutritional support at community pharmacies, while 56.1% of registered dietitians noted that they were not able to fully utilize their occupational abilities. Registered dietitians were divided into two groups: registered dietitians who answered that they were able to utilize their occupational abilities and those that answered they were not. The former group was more likely to ask pharmacists about patients' medication for dietetic consultation and to be asked to provide dietetic consultation to patients. The latter group was more likely to find difficulty in scheduling dietetic consultation.<h4>Conclusion</h4>Our results suggest that registered dietitians in community pharmacies have a greater explanatory ability than pharmacists concerning nutritional and dietary management for patients. It may be important for pharmacists to improve cooperation with registered dietitians by providing more opportunities for dietetic consultation.

Risk factors for severe neutropenia in pancreatic cancer patients treated with gemcitabine/nab-paclitaxel combination therapy

<h4>Aim</h4>Combination therapy with gemcitabine and nanoparticle albumin-bound paclitaxel (nab-paclitaxel), known as GnP therapy, significantly prolongs the survival of pancreatic cancer patients compared with gemcitabine monotherapy. However, it may cause severe neutropenia, requiring discontinuation of treatment. This study aimed to clarify the risk factors for Grade 3/4 neutropenia during GnP therapy.<h4>Methods</h4>Clinical data of pancreatic cancer patients who underwent GnP therapy at the Cancer Institute Hospital of the Japanese Foundation for Cancer Research from December 2014 to December 2016 were retrospectively collected. The relationship of Grade 3/4 neutropenia onset to laboratory values and patient background factors was investigated by multivariate logistic regression analysis.<h4>Results</h4>Clinical data of 222 patients were analyzed. Grade 3/4 neutropenia occurred in 118 patients (53.2%) in the first cycle of GnP therapy. Multivariate analysis identified low absolute neutrophil count (ANC), high total bilirubin (T-Bil), and low C-reactive protein (CRP) as risk factors for Grade 3/4 neutropenia. Age was not a risk factor. The incidence of neutropenia was 85.7% in patients with all three risk factors, but only 27.7% in patients with none of them.<h4>Conclusion</h4>Low ANC, high T-Bil, and low CRP may be risk factors for Grade 3/4 neutropenia in patients receiving GnP therapy, even if these laboratory values are within normal reference ranges. Patients with these risk factors should be carefully monitored for adverse events.

ドラッグストアまたはドラッグストア併設の保険薬局に勤務し、在学中にセルフメディケーション税制(以下、本税制)が施行されていた薬剤師を対象に、本税制に関する認知・見解実態把握調査(アンケート)を実施した。調査対象者は、男性138名(44.5%)、女性172名(55.5%)の合計310名であった。本税制の認知度と理解度について調査したところ、284名(91.6%)の調査対象者が「よく知っている(説明できる)」または「聞いたことはあるが、内容はよく知らない」と回答し、理解度に差はあるものの、ほとんどの調査対象者が本税制について見聞があった。これらの調査対象者は、主に「テレビ」、「雑誌本」、「インターネット」などのメディアを通して本税制に関する情報を得ていた。また、教育現場や職場で本税制について知った調査対象者も多かった。調査対象者の97名(31.3%)が本税制に関する講義を受講したと回答した。本税制の認知度を集計した結果、大学で本税制の講義を受けたことのあると回答した調査対象者において、「よく知っている(説明できる)」と回答した割合が顕著に高かった。今後、薬剤師として働くうえで、多くの調査対象者(86.4%)が本税制の知識が必要だと感じており、半数の調査対象者が本税制に関する情報を収集するうえで携帯アプリを活用したいと回答した。

健康セミナー参加者247名を対象に、セルフメディケーション税制に対する認知、理解度や関心などの実情に関するアンケートを実施し、本税制を取り巻く国民の実状と本税制の課題を明らかにすることを目的とした。調査の結果、過半数の調査対象者がドラッグストアを1ヵ月間に複数回利用しており、ドラッグストアを頻繁に利用している現状が示された。しかし、1年間のOTC医薬品の合計購入額が1万2千円以上10万円未満の調査対象者は11.8%(29人)と少なく、1年間のOTC医薬品の合計購入額が10万円以上の調査対象者はいなかった。セルフメディケーション税制の認知度について調査したところ、理解度に差はあるものの60%以上の調査対象者が本税制を認知していた。さらに、OTC医薬品の年間合計購入金額別に認知度を集計した。その結果、合計購入金額が1万2千円未満の調査対象者と合計購入金額が1万2千円以上の調査対象者で認知度は変わらなかったものの、合計購入金額が1万2千円以上の調査対象者のほうがセルフメディケーション税制について「知っている」と回答した割合が多かった。しかし、認知度に比べて申告件数は著しく少なかった。セルフメディケーション税制については、約50%(43人)が興味を示すとともに申告要件が揃っていれば控除を受けたいと回答した。

全国2000病院に看護師の抗がん剤注射剤取扱いに関する実態調査を行い、733施設より有効回答を得た。抗がん剤注射剤によるがん化学療法を実施していた385施設のうち、取扱う看護師が特定されているのは入院23.8%、外来50.6%、調製を看護師が行っているのはそれぞれ32.8%、24.2%、薬剤師が74.3%、77.7%であった。調製を看護師から薬剤師にシフトしていたのは6割で、95%以上が「良かった」と回答した。看護師の抗がん剤調製作業場所はナースステーションや外来処置室などの「クリーンルームではない部屋」、「通常の作業台」が最も多かった。抗がん剤注射剤の患者への投与やケアを看護師が行っているのは入院96.6%、外来97.1%、プライミングはそれぞれ89.4%、89.8%で、輸液ボトルの接続は90%以上であった。看護師が自主的に抗がん剤注射剤取扱いについてマニュアル作成を行っているのは45.8%、指導は38.1%であった。

著者らが運営する全国薬剤師間情報交換・研修システム「アイフィス」の登録薬剤師と、医師向け医薬品情報提供サイト「アイメディス」の登録医師に対してウェブサイト上でアンケートを行い、標題の認知度について調査した。医師は38名から回答が得られ、薬剤師は123名から回答が得られた。抗がん剤の添付文書に副作用として結膜炎や角膜炎が記載されていることを知っているか?という質問に対し「知っている」答えた人の割合は、医師が39%、薬剤師が33%であった。眼の副作用経験がある抗がん剤について尋ねたところ、医師・薬剤師とも「S-1」が最も多く挙げられた。

経口抗がん剤の調剤に関する実態調査

経口抗がん剤の調剤の実態を把握するため、アンケートを保険薬局36店舗の薬剤師(調査A)、インターネットの薬剤師間情報交換・研修システムに登録している薬剤師(調査B)に対して行い、それぞれ124名、39名より回答を得た。経口抗がん剤の特別な調剤(分包、粉砕、脱カプセル、簡易懸濁法など)の指示を受けた経験があるのは調査Aが30.6%、調査Bが97.4%で、その対応は「処方せん記載通りに調剤した」がそれぞれ76.9%、81.8%、「処方せん通りではないが調剤した」は13.5%、16.4%であった。疑義照会が行われたのは調査Aが52件中14件、調査Bが55件中14件で、これらの事例の殆どは指示された用量に適した規格が薬局の採用外であった。各事例で調製・調合する時に留意した点があったのは調査Aが55.8%、調査Bが52.7%で、その内容は「他薬へのコンタミネーション防止」43件、「調剤者の曝露防止」22件、「抗がん剤の性質への配慮」7件などであった。

離島における一般用医薬品のインターネット購入に関する意識調査　インターネットを使用しない居住者を対象として

長崎県五島市の離島住居者(16歳以上)から抽出した3819名に一般用医薬品の使用・購入に関するアンケート調査を郵送で行い、有効回答825名のうちインターネット非使用者522名を分析した。内訳は、本土との直接的な移動手段があり、薬局や薬店がある一次離島住居者が371名(男133名、女238名;A群)、直接的移動手段や薬局・薬店がない二次離島住居者が151名(男75名、女76名;B群)であった。一般医薬品の使用頻度は、2ヵ月〜1年に1回がA群33.2%、B群30.5%、1年に1回以下がそれぞれ27.5%、26.5%、使用なしが11.6%、21.2%であった。購入場所は「薬剤師がいて病院の薬を出す薬局」がA群41.5%、B群40.4%と最も多く、次いで「ドラッグストア・薬店」であった。今後期待する購入方法については「今のままで十分」がA群67.7%、B群59.6%で、インターネットで購入する必要性を感じたことがある、購入してみたいと思うのはそれぞれ4.9%、4.6%であった。

ヒト胎盤灌流法におけるフルボキサミンの胎盤透過特性について検討した。すべての灌流実験において、胎盤からの灌流液の漏出はほとんどなく、胎児側灌流圧は60mmHgを超えない値を示した。胎盤灌流実験における典型的な単純拡散マーカーであるアンチピリンの定常状態におけるTPTm,ss値は6.30%で、既報と同程度であった。ヒト胎盤灌流実験における母体側から胎児側、胎児側から母体側へのフルボキサミンの移行性はアンチピリンと同程度であった。非結合型薬物の胎盤組織/母体側灌流液濃度比から、フルボキサミンはアンチピリンよりも胎盤組織への移行(分布)が大きかった。washoutも実験における胎盤組織から灌流液中へのフルボキサミンの流出は速やかであった。

ウェブサイト「QLife」を介して、過去1年以内に医療機関の処方でNSAIDs貼付剤を使用した患者を募り、ウェブサイト上で残薬量や用途、副作用などについて無記名のアンケートを行い、618名(男性362名、女性256名、年齢10代〜80代)から回答を得た。処方診療科は整形外科73.8%、内科15.5%、外科7.6%の順で、処方されたパック数は中央値3パック、中に11パック以上の回答もあった。処方量は「ちょうどの量」41.1%、「かなり・やや多め」40.1%、「やや・かなり少なめ」18.8%であった。残薬の保存は「ある」61.2%、その量は中央値1〜2パック、残薬の用途は「同じ症状再発時に使う」71.2%、「家族が必要な時にあげる」49.7%、「別の症状で貼りたい時に使う」39.7%であった。副作用は「あると思う」32.8%、「ないと思う」24.3%、経験した副作用の症状は「かゆみ」40.0%、「かぶれ」39.5%など、使用期限のチェックは「する」58.9%、市販薬との違いは「よく効く」55.0%、「安い」80.0%であった。

全国2000病院を対象に抗がん剤調製環境についてアンケート調査を行い、1073施設より有効回答を得た。抗がん剤の注射剤混合業務を実施している360施設において、業務担当者は入院患者の場合は薬剤師70.8%、看護師10.8%、外来患者ではそれぞれ76.9%、10.3%であった。薬剤師が担当している施設の調製作業場所は「専用の調製室」47.9%で、「専用のクリーンルーム」「ほかの作業と兼用のクリーンルームではない部屋」がそれぞれ約20%であった。看護師が担当の施設では「ほかの作業と兼用のクリーンルームではない部屋」が86.7%で、具体的には「ナースステーション」が目立った。抗がん剤を調製する部屋に「保護具等を装着できる前室を備えている」のは30.3%、「流し台を備えている」は65.8%で、「室温のコントロールが調製室だけ独立している」61.4%、「生物学的安全キャビネットを備えている」75.8%、「調整作業時にケモシートを使用している」77.2%であった。

離島における一般用医薬品のインターネット購入に関する意識調査を実施した。1269件中、有効回答は825件で、16歳以上の五島市居住者2.2%にあたり、一次離島居住者648件、二次離島居住者177件であった。インターネット使用可能の離島居住者はインターネットによる一般用品の購入は行っていたが、一般用医薬品の購入を行っているものはほとんどなく、その必要性も感じていなかった。しかし、一部にはインターネットによる一般用医薬品の購入の必要性を感じている者もおり、インターネットによる一般用医薬品の購入を規制するのであれば、それに変わるシステムを早急に構築する必要があった。

離島地域のうち、インターネット環境がほぼ整備され、薬局・薬店のある地域の居住者を対象に、インターネットでの医薬品購入に関する実態や意識調査を実施して検討した。アンケート回答者の居住している離島を判定し、住民内訳は長崎県五島市66名(68%)、新上五島町6名(6%)、壱岐市25名(26%)であった。離島には薬局・ドラッグストア・薬剤師が存在した。回答者は19歳以下1名(1%)、20〜39歳63名(65%)、40〜59歳21名(22%)、60歳以上12名(12%)であった。75名(77%)は自宅にインターネット環境をもち22名(23%)はもっていなかった。75名の回答者はインターネットで医薬品以外の購入歴がある一方で22名は購入歴がなかった。インターネットで風邪薬などの医薬品を購入した住民は2名(2%)でごく僅かで、95名(98%)は購入したことがなかった。インターネットで医薬品を購入する必要性を感じたことがある回答者は全体の7%であった。

日本医薬品情報学会(JASDI)は医薬品情報学のあり方を提言することを目的に、ビジョン委員会を発足させた。当該委員会では医薬品情報学の教育が抱える問題点を、KJ法を用いて抽出、明確化することを目的に、2008年7月の第11回JASDI学会においてワークショップを開催した。抽出された大きな問題点は「医薬品情報の範囲が広すぎること」と「医薬品情報の定義とシステムが不明確なこと」であった。関連する問題点は歴史の浅さと独自性の欠如であった。また、研究の方法論が十分に確立していないこと、教育のシステム化ができていないことも指摘された。本ワークショップにより、医薬品情報学の抱える「本学問の定義づけ、体系化ができていない」という問題点が明確化すると共に、参加者に対しても有意な意識変化をもたらしたことが明らかになった。

臨床現場の薬剤師が患者の健康食品使用に対してどのように関わっているのか、また、健康食品情報の収集状況、被害情報の収集・アウトプットといった行動の現状を明らかにすることを目的として、著者らが2000年より構築・運営しているインターネットによる全国薬剤師間情報交換・研修システムi-PHISSを介してアンケートを行った。323名から回答が得られ、内訳は薬局薬剤師219名、病診薬剤師104名であった。処方箋受付時に患者の健康食品使用の有無を確認するか?という薬局薬剤師への問いに対して、「必ず確認する」と回答した人が29%、「必要に応じて確認する」が58%、「患者からの申し出がない限り確認しない」が12%であった。入院患者に健康食品使用の有無を確認するか?という病診薬剤師への問いに対して、「必ず確認する」と回答した人が44%、「必要に応じて確認する」が32%、「患者からの申し出がない限り確認しない」が10%であった。

ドラッグストアを訪れた消費者を対象に、青汁を飲む動機、青汁の安全性と有効性についての認識、サプリメント一般に関する情報の必要性など青汁の使用実態をアンケート調査した。2006年2〜3月に北海道、東北、関東地方のツルハの調剤併設ドラッグストア68店を訪れた患者、消費者563名を調査対象とした。青汁服用者は65歳以上の高齢者、降圧薬、抗アレルギー薬、糖尿病治療薬の長期間服用者で有意に多かった。33.2%は効果があると考えていたが、55%はわからないと答えた。青汁服用による体調不良は2.4%で起った。その他、毎日服用(55.5%)、注意事項不読(27%)などであった。青汁服用は長期医薬品服用の高齢者に多いことが判明した。青汁服用の効果と副作用についての証拠を集め、消費者に注意事項が確実に伝わる方法を検討する必要がある。

東京23区及び多摩地区の医療機関、保険調剤薬局に後発医薬品に関するアンケート用紙を配布し、臨床において後発医薬品が関係して生じたトラブルの事例を収集し、その要因を分析した。アンケートの総回答数は1148件(回収率64.9%)であった。後発医薬品が関係したインシデント・アクシデント事例として87例を収集した。医薬品の取り違えは49件と最も多く、その多くは薬名が類似しているものであった。次いで、先発医薬品と後発医薬品の適応症の違いが11件、使用感・服用感の違いが9件などであった。後発医薬品への切り替えに伴う効果の増強・減弱や有害事象の発生に関する訴えが5件収集されたが、これらの事象と後発医薬品への切り替えとの因果関係は不明である。

国民の健康食品への意識、使用動向を明らかにし、健康食品の安心・安全を確保するための方法を提案することを目的として、健康食品の使用実態や消費者・医療従事者の健康食品に関する意識を国内で調査・解析した過去の報告を収集し、統合を行った。十分な回答者数があり(N=300以上)、調査対象及び調査方法が明らかな9件のアンケート調査を選定し、使用目的、医療従事者と消費者との情報共有、情報の入手先と使用のきっかけ、健康食品の購入と入手先、健康被害の経験に関するデータを抽出し、解析した。その結果、健康食品に関して、健康被害をはじめとする多くの情報が見落とされ、また消費者がテレビやインターネット等で発信される不確かな情報を参考に健康食品を使用している可能性があるという現状が明らかになった。これらの結果を踏まえ、健康食品の安心・安全を確保するための全国ネットワークを提案した。

e-Learningシステム(アリセプト処方せんチェックe-ラーニング)を構築した。構築・公開したe-Learningシステムの利用実態を把握するとともに、システムに対するユーザーの意見を調査した。会員薬剤師5766例に対して、電子メールで送信した。198件の回答が得られた。薬剤師としての経験年数が10年以上の薬剤師が約62%を占めた。開局薬局が67.7%であった。自己研鑚システムとして有用と評価され、継続したシステムの公開が期待されていると考えた。また、他の医薬品に関しても同様なシステムを望む回答者が多く、要望が多かった医薬品を対象に計画的に作成し、公開してゆくことが望ましいと考えられた。

薬薬連携の中での疑義照会の現状と問題点を明らかにし、今後の疑義照会のあり方を探ることで適正な薬物治療を実施する上で有用な情報を構築することを目的にアンケート調査を行った。院外処方せんを発行する際に、処方鑑査を行っているかとの問いに関して、半数近くの47%が薬剤部の関与していないと回答した。院外処方せん発行後の〜次疑義照会先は薬剤部であるとの回答が74%を占め、一次疑義照会先を医師へ直接と決めている病院は12%にとどまった。保険薬局薬剤師の半数が、ほとんど近隣の医療機関からの処方せんを受けていると回答し、回答者の約98.5%が疑義照会の経験があると回答した。疑義照会先は、医師に電話(23.7%)、外来受付に電話、薬剤部に電話ほぼ同程度であった。疑義照会の内容としては、用法の不備、規格、単位の不備が大多数を占めた。

薬剤師に対するアンケート調査を実施し、モルヒネ製剤の剤形変更時の処方チェックや投与設計に関して、薬剤師がどのように対応したいと考えているか調査した。モルヒネ製剤の剤形変更時の処方設計を支援するための専用アプリケーションが必要であった。アンケート調査により、半数以上の薬剤師が、モルヒネ製剤の剤形変更時の処方チェックおよび投与設計を添付文書の用法・用量との照合、モルヒネ換算比・等鎮痛用量比との照合で行っていた。モルヒネ製剤の剤形変更時の処方チェックを薬物動態シミュレーション等に基づいて行いたいと考えているものの実践できていないことが示された。薬物動態シミュレーションに基づいた処方設計支援システムの構築は、薬剤師の現状の問題点を解決する有用な手段であることが示唆された。

服薬ノンコンプライアンス時の薬剤師の対応に関する実状と問題点

薬剤師間情報交換・研修システムに登録していた薬剤師4106名を対象に、電子メールを用いて患者のノンコンプライアンスへの対応に関するアンケート調査を行った。99.0%の薬剤師がノンコンプライアンス症例を経験し、そのうち75.5%が患者にノンコンプライアンス後の対処法を指示した経験があった。どのようにして対処法を決定したかについては、「次回から処方どおり服用するように指示した」が最も多く、「薬物動態シミュレーション等により血中濃度または薬効の経時的変化を計算して指示した」は2.6%のみであった。しかし、薬物動態シミュレーション等による投与設計で対応をしなかった薬剤師の57.5%が「対処法は適切だった」と回答した。対処法を決定する際の問題点については、「薬物動態シミュレーション等により対処すれば良いと考えたが、具体的な方法がわからなかった」が最も多く、問題点を解決するための要望については、「対処法について投与設計できるシステムが欲しい」が最も多かった。

薬剤師によるティーエスワンに関する疑義照会の実態調査を行った.ティーエスワンの処方箋を応需した薬剤師において禁忌や他のフッ化ピリミジンからの切り替え時の注意などに理解がなく疑義照会に至らない処方箋チェックがあり,薬剤師への的確な医薬品情報提供の必要性が示された.ティーエスワンの処方を応需し,より多くの薬剤師に医薬品情報を効率よく提供する方法として,より実践的に処方箋応需業務を支援することを目的に,インターネットを介した薬剤師を対象とするWeb e-learningシステム(EL)を構築し評価を加えた.ELを使用した薬剤師のほとんどが参考になったと回答しており,ELは医薬品情報を提供するうえで受講者の満足度を得られる有用なシステムであることが証明でき,時間的自由度,学習効果,受講者の満足度とも良好であった
