Preparing for a paradigm shift in aging populations: listen to the oldest old

Purpose: Current healthcare systems are not suitable for serving future societies in which the oldest old are commonplace. The objective of this study was to understand what the oldest old care most about in their daily lives. Methods: Semi-structured in-depth interviews and thematic analysis were used. Face-to-face interviews were conducted in 17 elderly residents (≥ 95 years) of Arakawa-ku, Tokyo, Japan from July to November 2017. Results: Three themes emerged from the interview responses: “unshakable beliefs and social ties,” “natural acceptance,” and “my day-to-day life with precious moments.” The oldest old strongly believe in diligence and compassion and maintain strong relationships with people around them. Despite their small social networks, they are concerned about future society. They accept their selves and their lives, including their impending deaths. Despite their functional decline, they control their lives by making very small decisions. They live on a moment-to-moment basis, cherishing simple events. Conclusion: Maintaining autonomy through making small decisions and enjoying small pleasures are important to the oldest old. Understanding the needs of the oldest old is the first step towards developing optimal geriatric care for an aging population.

Regular group exercise contributes to balanced health in older adults in Japan:A Qualitative study.

Background: While community-wide interventions to promote physical activity have been encouraged in older adults, evidence of their effectiveness remains limited. We conducted a qualitative study among older adults participating in regular group exercise to understand their perceptions of the physical, mental, and social changes they underwent as a result of the physical activity. Methods: We conducted a qualitative study with purposeful sampling to explore the experiences of older adults who participated in regular group exercise as part of a community-wide physical activity intervention. Four focus group interviews were conducted between April and June of 2016 at community halls in Fujisawa City. The participants in the focus group interviews were 26 older adults with a mean age of 74.69 years (range: 66-86). The interviews were analysed using the constant comparative method in the grounded theory approach. We used qualitative research software NVivo10® to track the coding and manage the data. Results: The finding 'regular group exercise contributes to balanced health in older adults' emerged as an overarching theme with seven categories (regular group exercise, functional health, active mind, enjoyment, social connectedness, mutual support, and expanding communities). Although the participants perceived that they were aging physically and cognitively, the regular group exercise helped them to improve or maintain their functional health and enjoy their lives. They felt socially connected and experienced a sense of security in the community through caring for others and supporting each other. As the older adults began to seek value beyond individuals, they gradually expanded their communities beyond geographical and generational boundaries. Conclusions: The participants achieved balanced health in the physical, mental, and social domains through regular group exercise as part of a community-wide physical activity intervention and contributed to expanding communities through social connectedness and mutual support. Health promotion through physical activity is being increasingly emphasized. The study results can help to develop effective physical activity programs for older adults in the community.

Nurses’ perception of decision-making about cancer treatment and fertility preservation by young breast cancer patients

本研究は、若年乳がん女性の治療と妊孕性に対する意思決定支援について、看護師がどのように認識し、実践しているのかを明らかにするために、フォーカスグループと内容分析による質的研究を行った。研究参加者は、若年乳がん患者に携わる専門看護師と認定看護師15名であった。結果として、＜不確かなライフプランの支援＞がコアカテゴリーとして導かれ、3つのカテゴリー〈不確かな将来を歩むための準備〉〈ライフプランの不確かさに伴う葛藤への支援〉〈女性の決断に沿った最善のケア〉が含まれた。看護師は、若年女性ががん治療と妊孕性の課題に伴う葛藤から納得や折り合いをつけたライフプランを支援し、そして女性が決断した不確かな道に歩み出すことを支え、最善を尽くすことの重要性を認識し、実践していた。さらに、看護師は、女性の真の願いを叶えるために組織を超えた多職種の連携、協働や最善のケアを提供することの重要性を認識していた。

Nurse Counseling for Physical Activity in Patients Undergoing Esophagectomy

Strong evidence exists regarding multiple benefits of physical activity among cancer patients. Patients undergoing esophagectomy received counseling for physical activity by a nurse and instructions to keep a diary of physical activities before surgery, followed by 2 counseling sessions after surgery. Physical activity, body mass index, psychological distress, and quality of life were measured at baseline, 2–4 weeks, and 3 and 6 months after discharge. Of 29 participants (mean age = 65.9 years), 72.5% underwent thoracoscopic esophagectomy in the prone position with 3-field lymphadectomy. The results of the International Physical Activity Questionnaire indicated that postoperative physical activity returned to nearly preoperative level, and the number of inactive patients gradually decreased by half at 6 months after discharge. Quality of life scores, except those for role function and social function, tended to return to the baseline at 6 months after discharge. Psychological distress measured by the Kessler 6 remained lower than the baseline, whereas the mean score of body mass index gradually declined after discharge. Patients undergoing esophagectomy and receiving nurse counseling can return to preoperative physical activity level in their daily lives after surgery. Nurses are critical to the physical management and psychological support of patients undergoing chemotherapy, which is a vulnerable time for many. This article presents the results of a qualitative study intended to explore the experience of Japanese patients with breast cancer during chemotherapy, including the finding that participants created personal safety nets in physical, emotional, and social contexts that helped them to gain confidence in their ability to exert control over their lives. Understanding each patient’s personal safety net allows nurses to support their patients in maintaining and improving their function and well-being. At a Glance • Patients with cancer create personal safety nets in physical, emotional, and social contexts during chemotherapy. • Nurses can reduce patients’ distress, strengthen their safety nets, and empower them to lead meaningful lives. • Nursing care should serve as a safety net for patients undergoing chemotherapy and promote the feeling of safety.

Personalized medicine, the tailoring of prevention and treatment, is the future of routine clinical practice. This approach has started to appear in genetic testing for predisposition to hereditary breast and ovarian cancer (HBOC). We explored how breast-care providers perceived HBOC risk management, using grounded theory. This study found that the frontline healthcare providers perceived HBOC risk management as still being neglected in breast cancer care. Emerging challenges included treatment priority, hesitancy to deal with sensitive issues, easily missed risks, genetic data not being shared among multidisciplinary professionals, and patients being lost to follow-up. Oncology nurses are ideally placed to facilitate communication and utilization of genetic information among multidisciplinary professionals. Specialized outpatient clinics need to be established to follow up individuals at high risk. There is a need to create a system to meet the future demands of personalized medicine in nursing practice. © 2013 Wiley Publishing Asia Pty Ltd.

Repair of the threatened feminine identity:Experience of women with cervical cancer undergoing fertility preservation surgery.

The underlying reasons behind suboptimal reproductive results after successful fertility-preserving surgery have not yet been fully revealed. Objective: The objective of this study was to explore the experience of fertility preservation with radical trachelectomy from the perspective of women with cervical cancer. Methods: We conducted interviews with women with cervical cancer who underwent radical trachelectomy using a Grounded Theory methodology with a theoretical framework of symbolic interactionism. Results: Our findings articulate a process in which feminine identity is first threatened by a diagnosis of cancer, then repaired by fertility preservation with radical trachelectomy, and finally reconstructed after the surgery, through interactions with self, others, and external events in women with cervical cancer. Feeling incomplete as a woman because of the loss of the uterus was a critical factor in the women's feeling that their feminine identity was threatened. Thus, fertility preservation was significant for these women. The meaning of fertility preservation varied among the women, and their life perspectives were therefore distinct after the surgery. Conclusions: Women with cervical cancer who undergo radical trachelectomy experience an identity transformation process, and child bearing is not the only expected outcome of fertility preservation. Implications for practice: Nurses should coordinate care through the cancer trajectory. Background: The Fatigue Barriers Scale (FBS) is an instrument to identify patient-derived barriers to fatigue management. Objective: To assess the reliability and validity of the Japanese version of the FBS (JFBS). Methods: Participants were 112 cancer patients. Reliability was determined through internal consistency using Cronbach's alpha coefficient and through test-retest reliability using Spearman's intra-class correlation coefficient (ICC). Construct, convergent, criterion-referenced, and concurrent validities were assessed. The Cancer Fatigue Scale (CFS), Patient Fatigue Knowledge Tool (PFKT), and Kessler 6-item distress scale (K6) were used for comparisons. Results: Cronbach's alpha coefficients were 0.74, 0.78, 0.72, and 0.54 for the JFBS total and for each subscale, respectively. All ICCs exceeded 0.65 (P<0.05). The JFBS had a weak negative correlation with the PFKT and a positive correlation with the CFS. The K6 scores showed weak positive correlations with the JFBS total and fatalism subscale scores. Conclusions: The JFBS can be used to measure fatigue barriers in Japanese people with cancer. © 2013 MA Healthcare Ltd.

The need for a nursing presence in oral chemotherapy

As cancer treatment shifts from IV to oral chemotherapy, patients have less contact with nurses and face the increased responsibility of maintaining their own health care. The authors conducted focus group interviews with 18 oncology nurses using the grounded theory approach to explore the nurses' perceptions of current practices and ideas regarding opportunities to improve nursing practice for patients receiving oral chemotherapy. The nursing presence is becoming invisible and, therefore, these patients are isolated more in current practice. "The need for a nursing presence" emerged as a core category. Nurse involvement in patient entry into oral chemotherapy was perceived as important for identifying patients at risk for nonadherence. In their partnership with patients, nurses should be attentive, connect with patients, elicit patients' unmet needs, and provide committed patient support. Rather than the traditional reactive approach, proactive patient care is required. In addition, nurses are expected to coordinate patient care and facilitate interpersonal relationships among healthcare providers. Coordinated proactive care leads to predictive care to meet the future needs of patients, including the prevention of adverse events. The roles and responsibilities of nurses in oral chemotherapy must be clarified so that the nursing presence is clear to patients and other healthcare providers. © Oncology Nursing Society.
