音楽家の脳機能の解明を目的とし、音楽リズム処理機能と精神疾患の関連性、音楽リズム処理や和音処理機能の神経基盤、音楽演奏評価と視覚情報の関連性、音楽家の身体運動制御やドラマージストニアの身体症状の解明等、以下(1)-(7)の研究を推進した。(1) 前年度に引き続き、研究分担者と連携してマルチモーダル脳機能イメージング手法(核磁気共鳴画像法 [MRI]、核磁気共鳴スペクトロスコピー法 [MRS]、経頭蓋磁気刺激[TMS]-高解像度脳波[EEG]の同時計測法 [TMS-EEG])を用いた実験調査を継続して推進した。(2) 精神疾患、特に統合失調症患者の音楽リズム機能をハーバードビート評価テスト（H-BAT）を用いて評価し、治療抵抗性、精神症状、認知機能との関連性を調査し、患者の認知機能の低下が音楽リズム機能の低下と関連していること、及び、治療抵抗性によって音楽リズム生成機能に差異があることを明らかにした。(3) 音楽リズム機能とMRSで評価した脳内グルタミン酸濃度との関連性についてデータ分析を進めた。(4) 協和音の嗜好性と安静時脳機能ネットワークの関連性について、左側坐核ー右上頭頂回、前帯状回ー後上側頭回の安静時脳機能ネットワークが協和音の嗜好性に関連している可能性を得た。(5)演奏評価判断中の視聴覚情報処理過程について調査し、演奏評価判断時の視聴覚情報処理過程が音楽経験によって異なる可能性を得た。(6)ドラマー・パーカッショニストのリズム生成機能や音楽家のジストニア症状とリズム同期誤差・筋活動の関連性について調査・分析した。(7)その他、音楽リズムと身体運動の共鳴をテーマとし、音楽リズムと身体運動に関する研究が人類の起源・脳・身体機能の解明にもたらす意義について、総説・解説記事をまとめて発表した。

Elucidation of the pathophysiology of prefrontal cortex in patients with treatment-resistant depression using transcranial magnetic stimulation neurophysiology

In this study, 30 patients with medication-resistant depression were strictly enrolled in neurophysiological experiments using TMS combined with high-resolution EEG. In addition, age- and sex-matched 30 healthy controls with depression were recruited and TMS-EEG experiments were conducted. Specifically, GABA(A) receptor- mediated neurophysiological function, glutamate NMDA receptor-mediated neurophysiological function, and neuroplasticity in the dorsolateral prefrontal cortex (DLPFC) were measured with combined TMS-EEG method by applying short-interval intracortical inhibition (SICI), intracortical facilitation (ICF), and paired associative stimulation (PAS), respectively. As a result, it was suggested that glutamatergic excitatory function as indexed by ICF may be relatively elevated and neuroplastic function as indexed by PAS may be decreased in patients with treatment-resistant depression compared with healthy controls in the DLPFC.

In schizophrenia, basal ganglia glutamate concentrations are elevated, glutamate and GABA concentrations in the middle cingulate gyrus are decreased, basal ganglia glutamate concentrations may decrease with disease progression and antipsychotic treatment, and middle cingulate gyrus GABA concentrations may increase with the onset of We found that the mid-cingulate gyrus GABA concentrations may increase with the onset of illness. Next, we reported increased glutamate concentrations in the middle cingulate gyrus in treatment-resistant schizophrenia. However, while significant differences were found between treatment-resistant schizophrenia and healthy subjects, no statistically significant differences were found between treatment-resistant schizophrenia and schizophrenia in remission, a finding that suggests heterogeneity of the condition within each group. The findings regarding elevated GABA concentrations in treatment-resistant schizophrenia are under review.

To date, only four studies have examined glutamatergic neurometabolite levels, using proton magnetic resonance spectroscopy (1H-MRS), in patients with treatment-resistant schizophrenia (TRS). However, findings have been inconsistent. Glutamate plus+ glutamine (Glx) levels were assessed in the dorsal anterior cingulate cortex (dACC) and caudate. Glx levels were compared in TRS patients versus non-TRS patients, and healthy controls (HCs), using 3T 1H-MRS (PRESS, TE=35ms). A total of 95 participants were included (TRS=29, non-TRS=33, and HCs=33). dACC Glx levels were higher in the TRS group versus HCs. No group differences were identified in the caudate. Our results suggest that higher Glx levels in the dACC may reflect the difference in disease category between TRS and non-TRS.

We have established a randomized controlled trial (RCT) protocol of repetitive transcranial magnetic stimulation (rTMS) for the medication-resistant patients with major depression, controlled with a sophisticated sham rTMS system. The present RCT design could make it possible to compare different rTMS protocols directly during the acute phase and to examine a prophylactic effect of rTMS during the maintenance phase. As this RCT has not been completed so far, the final analyses, including neurobiological changes induced by consecutive rTMS sessions, are going to be done after the RCT completion.
